  Apicoectomy is a surgical method to promote periapical healing by scraping away periapical diseased tissue and removing the infected root tip, treating the apical stump, and depositing calcified material by using blood clot mechanization. Additional or other procedures such as apical scraping, fractured apical extraction, and root canal inversion filling are also performed on the basis of apicoectomy.  Apicoectomy is a procedure to remove the root tip of a tooth and scrape away the diseased periapical tissue. It is suitable for cases where root canal treatment fails and the root canal filling material cannot be removed; root canal is bent and narrow; root canal instruments are broken in the root canal and blocked; root tip fracture has formed chronic periapical inflammation; chronic periapical inflammation combined with overfilled root canal filling material that is difficult to remove. Generally, it is only used for anterior teeth and premolar teeth, and molar teeth can be treated as appropriate for anatomical conditions. Pre-operative x-rays should be taken to understand the root morphology, location, relationship with adjacent tissues, extent of bone destruction, and root canal treatment. It is better to have root canal filling before surgery, or to have root canal filling done at the same time.
