What are the general treatments for glaucoma?

  First of all, glaucoma patients and their families should be clear about one thing: Glaucoma cannot be completely cured. Whether we treat it with medication or surgery, the goal is to lower the intraocular pressure in order to avoid the damage to visual function caused by high intraocular pressure. Therefore, the goal of glaucoma treatment and the best outcome is to stop the loss of visual acuity and visual field.  There are several main treatments for glaucoma: 1. medication. If the IOP can be controlled with a few medications to hit the treatment goal, this is ideal and there is no need for surgery. However, the worst thing about this treatment is that it requires long-term eye medication, which is like eating. At the same time some medication, over time there will also be drift phenomenon, that is, the drug effect is not ideal, need to change and adjust the medication, etc.. At the same time long-term use of eye drops, due to the preservatives and other ingredients in the eye drops, can also lead to dry eye and other pathologies, this situation is very common in patients with glaucoma.  2. Laser treatment. In some patients with conditions such as iris bulging, a YAG laser can be applied to open up the root of the iris to solve the problem.  3. Trabeculectomy. This procedure is the most commonly used of all surgeries for glaucoma, especially now with the combination of mitomycin, with a success rate of up to 80-90% for the first surgery. However, due to individual differences and surgical skills, some patients may have postoperative follicular incompetence or leaking follicles, which can be reoperated. In conclusion, this procedure is simple and easy to perform, and is now the routine and preferred choice for glaucoma surgery.  4. Glaucoma drainage valve implantation. The advantage of this surgery is that the effect of lowering IOP after surgery is fast, but the disadvantage is that the drainage valve is easy to wrap for a long time and lose its drainage function, and at the same time, low IOP may appear just after the surgery, which may cause pulse dehiscence and other situations. So I personally think that I do not recommend this surgery for the first time, if the effect is not satisfactory for many times repeatedly, and some special types of glaucoma, you can consider this type of surgery.  5. Other. There are some other procedures, such as Schiemm canal molding, ciliary body photocoagulation, ciliary body cryosurgery, etc. are less applicable.
