How does Chinese medicine treat gynecomastia?

In recent years, the incidence of gynecomastia has been on the rise as people’s standard of living has improved and their dietary habits have changed. Gynecomastia belongs to the category of “breast scrofula” in Chinese medicine. In Chinese medicine, men’s nipples belong to the liver and the breasts belong to the kidneys. If the emotions are not regulated, the liver is depressed; or if the body is old and weak, the liver and kidneys are deficient; or if the congenital endowment is insufficient, the impulse is not regulated; or if the liver is injured by external evil, the liver is not drained, which can lead to the loss of nourishment of the meridians and blood, and thus blood stasis and phlegm clotting block the breast channels and cause the disease. Chinese medicine has a profound understanding of the etiology and pathogenesis of “breast scrofula”, and has accumulated rich clinical experience in the treatment of this disease by combining disease and evidence. Chinese medicine believes that the pathogenesis of “breast scrofula” is closely related to the liver and kidney. The liver meridian surrounds the pubic apparatus and passes through the stage door point under the breast and up through the diaphragm and the wakeful ribs; the testes belong to the liver and the kidneys, and the kidneys collect sperm and are responsible for reproduction and development. The Compilation of External Medical Cases – Breast and Axillary Ribs says: “Men’s breasts belong to the kidney, why? Men with gas as the main, women with blood as the first, foot Shao Yin kidney veins through the bladder, its straight from the kidney through the liver and diaphragm, into the lungs, the water a little real Yang, straight through the three yin above. The water does not contain the wood, wood gas is not comfortable, the true yang can not reach up. Nodules in the breast, gas depression? Although the cloud of liver disease, its origin is in the kidney.” The importance of the kidney in the pathogenesis of breast scrofula is emphasized even more. The etiology and pathogenesis of this disease have been described by many medical practitioners over the ages, each with its own focus. Modern medical practitioners have based on ancient literature, combined with rich clinical experience, and have played a lot. According to Lu Deming, the nipples of men belong to the liver, the breasts belong to the kidneys, and the punching point belongs to the liver and kidneys, and the root of the punching point is the kidneys. According to Prof. Lu, the kidney and liver are of the same origin, and the essence and blood are mutually transformed, making them the organs of mother and child. The liver’s function of blood collection and drainage depends on the warmth and support of kidney energy. If the kidney Qi is not sufficient due to lack of innate endowment, or if the kidney Qi is deficient due to old age and weakness, or if the kidney is depleted due to long term illness, the kidney is not nourished, or if the kidney is depressed due to long term use of products that harm the liver, resulting in liver deficiency and liver stagnation, or if the kidney is deficient in essence due to kidney deficiency, the yin and yang of the kidney can become unbalanced, the kidney Qi is not sufficient, and the flushing of the kidney is not regulated, so that the liver cannot nourish the liver wood. In this case, the disease is caused by the stagnation of Qi, blood stasis and phlegm in the breast lobe. Therefore, the root of the disease is the deficiency of kidney qi, the loss of regulation of the flushing and the loss of nourishment of the liver, and the symptoms of the disease are the stagnation of liver qi, the loss of healthy movement of the spleen, and the stagnation of qi with phlegm and stagnation of blood. (Wang summarized clinical cases and concluded that Qi stagnation and phlegm stagnation are the basic pathogenesis of breast scrofula. The onset of the disease can be due to long-term mental depression, or trauma, unhappy emotions, liver qi stagnation, refining liquid into phlegm, phlegm, qi, and fire intertwining in the breast ligaments, or due to a deficiency of yin and yang in the kidneys of men, unable to nourish liver wood, loss of nourishment of liver wood, lack of relaxation of wood qi, loss of power of drainage, stagnation of qi and phlegm, and loss of harmony in the breast ligaments. It can also be caused by external warmth, heat, epidemic and toxins, or by long-term use of drugs that injure the liver due to other diseases, resulting in the loss of proper drainage and incompetence of the milk channels. The two words “phlegm” and “qi” are always the same. According to Xu, the onset of breast scrofula may be due to liver qi stagnation, qi depression turning into fire, refining liquid into phlegm, phlegm and qi interlocking, and the loss of harmony between the breast ligaments; or due to various reasons resulting in liver and kidney deficiency, the essence and blood cannot fund the flushing and the flushing and the flushing and the flushing and the flushing pulse are incompetent, and phlegm and stagnation clog the breast ligaments. The treatment is to de-stagnate liver and Qi, resolve phlegm and disperse knots. The treatment is to tonify the liver and kidney, dissolve phlegm and soften hardness, and the formula is Liu Wei Liu Huang Tang. Liu Wei Di Huang Tang with Angelica sinensis, Bai Shao, Chuan Bei and Oyster. Shi also divided breast scrofula into 2 types: the type with deficiency of kidney yang, treated with Xian Mao, Xian Ling Spleen, Angelica sinensis, Bai Shao, Bacopa monniera, Cinnamon, Silkworm Lobelia, Semen Nelumbo and Orange Kernel; the type with liver stagnation and phlegm coagulation, treated with Chai Hu, Qing Pi, Angelica sinensis, Bai Shao, Yu Jin, Semen Nelumbo, Bayberry, Xiang Shen, Silkworm Lobelia, Orange Leaf and Orange Kernel. Summary Chinese medicine has been studied on the disease of breast scrofula for an ancient time and has a profound understanding of the etiology and pathogenesis of the disease, and has the advantages of combining disease identification and evidence identification in treatment, and flexible and diverse treatment methods, and Chinese medicine has accumulated rich experience in the treatment of this disease, and there are many clinical experiences worth studying and learning from, and a large number of theories and prescriptions for our reference. Although the clinical treatment of this disease in TCM has been reported repeatedly, it is mostly limited to the discussion of etiology and pathogenesis, the summary of evidence-based treatment or the report of single prescriptions and experimental prescriptions, and lacks strict design. We suggest that clinical research and experimental studies should be strengthened in future work to make full use of the knowledge of various related disciplines to further analyze the principles of TCM treatment of this disease, to screen out the exact and effective TCM compound prescriptions, and to find out the rules of TCM treatment of this disease for clinical guidance.
