An axial position of 180 degrees often means that the astigmatism is in the horizontal axis. Axial position 180 degrees and axial position 90 degrees can be converted to each other, however, provided that the patient is most comfortable with them, so make sure that the patient tries on the glasses before making the conversion and uses whichever axial position is most comfortable. If the wrong axis is used, glare, dizziness, and other discomfort can easily occur. However, it is best to have a dilated eye exam before fitting to see the true refractive state and to see if there is myopia, hyperopia, or astigmatism. If the astigmatism is relatively large, it is best to check the corneal topography to rule out the possibility of cone corneas. It is very crucial not to make a mistake in the axial position, once the wrong prescription, the glasses lenses may need to be re-fitted.
