I. Overview 1. Characteristics: Lymphangioleiomyomatosis is a benign misshapen tumor. They contain interconnected lymphatic vessels and cystic spaces, most of which are less than 5 mm in diameter. Two-thirds of them have manifestations at birth, and most of them occur within 2 years of age. Some cases are diagnosed prenatally. Lymphangioleioma rarely heals on its own, and most of them do not recede on their own, but continue to grow and invade adjacent tissues, including subcutaneous tissue. 2. Site of occurrence: It can occur all over the body, 75% in the head, neck and axilla, most of them grow exophytically, some cases infiltrate deeply; it can also occur in liver, spleen, intestine and other organs. Classification 1. Simple lymphangioleioma can occur in all parts of the body and manifest as a single blister of no more than one centimeter.2. Spongiotic lymphangioleioma is the most common, varying in size, in the head and neck, extremities, trunk, floor of the mouth and mediastinum.3. Cystic lymphangioleioma occurs mostly in the neck. Some of them are mixed with dilated capillaries or veins and are called lymph angiomas. Although lymphangioleioma is a benign lesion, its biological behavior can cause harm to human body.1 Infection: it can cause lymphangitis, cellulitis and even sepsis.2 Compression: it can cause symptoms in the corresponding area. If it occurs in the head and neck, invades the floor of the mouth, trachea or mediastinum, it can cause obstruction of the respiratory tract and difficulty in childbirth.3 Affects the function: Invades the tongue, resulting in the inability to close the mouth. To differentiate it from lymphedema, usually ultrasonography can differentiate it. V. Treatment Lymphangioleiomyomatosis is usually treated by injection therapy or surgery, depending on the condition and the doctor’s own skills. 1.Injection therapy Different types of lymphoma respond differently to injection, while cystic lymphangioleiomyolymphoma has good results and cavernous lymphangioleiomyolymphoma has poor response. Since injection therapy is easier to operate and less tissue destruction, surgical damage and related complications can be avoided. In addition, it is difficult to completely remove the tumor by surgery, so some people regard injection as the first choice for cystic lymphangiectasia. 2.Surgical treatment Indications for surgery: affecting the appearance or disfigurement; huge tumor; chronic lymphatic leakage; repeated infection. Surgical resection is the main treatment for lymphangioleiomyoma, and the infection should be controlled first when combined with infection. The actual extent of lymphangioleioma is difficult to estimate accurately before surgery, and it is often difficult to remove it completely during surgery. The important nerves, blood vessels and other structures in the neck should be carefully dissected to prevent damage to facial nerve palsy and lingual nerve, recurrent laryngeal nerve and phrenic nerve. 3.Waiting for self-healing: Some characters have a tendency of natural regression of few lymphangioleiomas, which can be observed and limited to smaller limited lymphangioleiomas, which do not affect function and appearance cases.
