PPH Hemorrhoid Surgery Aftercare Considerations

Dietary care for postoperative constipation or diarrhea: After the surgery, patients are generally required to start defecation around 48 hours, once a day, and the stool should be soft and mushy, in order to avoid injury and pain aggravation caused by friction when the stool is discharged through the trauma. If there is no bowel movement for 3 days after the operation, patients should be instructed to increase the amount of food containing vegetable fats, such as sesame, broth, etc., or take a small amount of sesame oil or honey with boiled water at night before going to bed, and after the above dietary treatments, the bowels can be discharged in general. If it can not be discharged, can be used to open the small enema. If diarrhea occurs after surgery, instruct the patient to eat light, easy to digest food, pay attention to dietary hygiene, fasting cold. And can drink strong tea has to facilitate urination and stool formation, if necessary, can use antidiarrheal drugs. Difficulty in postoperative defecation dietary care: the day after surgery or the second day, mostly due to the effect of anesthesia, surgical stimulation, wound pain or dressing compression, causing reflexive bladder neck sphincter pain, spasm caused by postoperative urinary difficulties. Such patients should drink strong tea or sugar water to increase the volume of urine, stimulate the bladder, enhance the desire to urinate, but also can relax the dressing compression wound, prompting urination. Those who still cannot urinate should be catheterized as prescribed by the doctor. Dietary structure and hemorrhoid healing has a close relationship. If the food is too fine and too refined, it will cause intestinal peristalsis to slow down, cause constipation, and lead to varicose veins in the anorectal area; if too much chili pepper or pepper intake in the food, it will stimulate the anorectal area, causing blood vessel congestion and expansion in the anorectal area; if the diet intake is too much or too much coarse food, it is easy to cause too many bowel movements after the operation and cause wound infection; if the diet before and after the operation is well regulated, it not only makes the operation smoothly, but also plays a role in the repair of the wounds. If the diet is properly regulated before and after surgery, it can not only make the surgery go smoothly, but also play an important role in the repair of the wound, reduce the pain, favor the wound healing, and at the same time prevent and reduce the occurrence of postoperative complications. (1) Diet: liquid or semi-liquid diet on the first postoperative day, and normal diet on the next day; (2) defecation: control defecation for 1 day after surgery, and oral liquid paraffin oil or hemp seed pills to soften the stool, and sitz bath after defecation. There is no need to change the medicine, and it is not necessary to incorporate suppositories into the anus, etc.; (3) the use of antibiotics: the general postoperative prophylactic application of antibiotics for 1-3d to prevent anastomotic infections; (4) the observation and treatment of complications: if the surgical operation is standardized, there is usually no near-term complication, and on the contrary, there are bleeding, anal pain, and far-away anastomotic stenosis, etc.. Therefore, closely monitor the blood pressure and pulse within 24h after operation, make rectal palpation after l week, dilate the stenotic segment if necessary, and hang the wire incision if the stenosis is serious.
