Do you have to remove epididymitis in old age?

Epididymitis in old age does not necessarily need to be removed, usually applying general treatment, antibiotic treatment, surgical treatment and so on.
1. General treatment: rest, support the scrotum, analgesia, avoid sexual intercourse.
2. Antibiotic treatment: use antibiotics if infection is suspected. Before using antibiotics, urine samples should be taken for bacterial culture and drug sensitivity test, and chlamydia test should be performed routinely. The treatment of choice is usually azithromycin and cephalosporins to cover gonorrhea and chlamydia. Sensitive antibiotics are later selected based on culture results, and antimicrobials are usually used for 2 to 4 weeks.
For cases caused by enteric organisms (e.g., Escherichia coli), ofloxacin or levofloxacin is recommended.
3. Surgical treatment: For purulent epididymo-orchitis, we can choose epididymo-seminal cord peritomy and reduction, abscess incision and drainage or epididymectomy. For testicular infarction or larger testicular abscess, orchiectomy is feasible.
