Renal fibrosis is a common pathological manifestation of end-stage renal disease due to various causes, including glomerulosclerosis and renal interstitial fibrosis. Numerous studies have shown that renal interstitial fibrosis (RIF) is a common pathway for the progression of various chronic kidney diseases to end-stage renal failure, and it is the main factor determining the progression of kidney diseases. According to the clinical manifestations of end-stage renal failure in modern medicine, Chinese medicine believes that it belongs to the categories of “Guan Ge”, “Edema”, and “Deficiency Labor”. However, there is no unified understanding, especially the theoretical understanding of the combination of Chinese and Western medicine. This hinders the choice and research of Chinese medicine on the treatment plan of renal tubulointerstitial fibrosis. The author has devoted himself to the basic and clinical work of combining Chinese and Western medicine to prevent and control renal tubulointerstitial fibrosis for many years, and has certain insights into the characteristics of the pathomechanism and treatment of renal tubulointerstitial fibrosis. Based on the evidence of Chinese medicine theory and ancient literature, the author suggests that chronic kidney disease/renal fibrosis should belong to the category of “dirty impotence” in Chinese medicine. The treatment of renal fibrosis from “renal impotence” can open up a new way for clinical treatment of renal fibrosis. 1. Impotence and visceral impotence literature, impotence that is, impotence L, according to the “People’s Republic of China traditional Chinese medicine industry standards”, “Chinese medicine diagnosis and efficacy standards”. Chinese medicine industry standards? Chinese medicine diagnosis and efficacy standards”, also known as “impotence disease”. Definition: “impotence disease is caused by evil heat injury to the fluid, or qi and yin insufficiency, and lead to meridian loss of nourishment, to limb weakness, meridian relaxation, or even muscle atrophy or paralysis as the main manifestation of the limb disease”. Polyneuritis, acute myelitis, progressive myasthenia gravis, myasthenia gravis, and other central nervous system infections and neuronal lesions to the sequelae of concurrent flaccid paralysis, etc. are all in the category of impotence in traditional Chinese medicine [1]. Represented by “Suwen? Impotence theory” as a representative, the “Neijing” according to the different symptoms of impotence and the five viscera corresponding to the name of “five viscera impotence”, namely, impotence L, vein impotence, tendon impotence, meat impotence, bone impotence. It is also clearly pointed out that bone impotence is a disease of the kidney. Excessive exertion, feel the external evil can lead to bone impotence. Such as “there are long-distance labor, meet the great heat and thirst, thirst is the yang qi internal logging, internal logging is hot in the kidneys, kidneys of the water organs, this water does not prevail over fire, the bone wither and marrow, so the foot does not let the body, the hair for the impotence of the bone”. The lower scripture” said: bone impotence, born of great heat. Su Wen” seven treatise also mentioned in many places climate anomalies, the four seasons of the evil invasion of the body can be directly people “less gas bone impotence”. Kidney color black, the main bone, so bone impotence has “waist and spine do not lift, bone withered and marrow reduction”, “yellow color”, “tooth haggard” and so on the manifestations of kidney lesions (“Su Wen? Impotence”) [2]. However, the “five viscera impotence” is still the actual skin, veins, muscles, flesh and bones of the five impotent, to the title of the combined viscera, intended to emphasize the five viscera heat impotence of the disease mechanism. We further evidence-based on the ancient literature found that the “dirty impotence” is really there, and is different from the above “five viscera impotence”. Its clinical manifestation is based on the malfunction and decline of the internal organs themselves. The word “impotence” is the same as “委”””萎”. Guangyun” said “atrophy, wilting also”, suggesting that the ancient said impotence, “dirty impotence” is intended to reflect the characteristics of tissue and organ atrophy, withered. The Guangya Shuozheng, Volume 1, “Commission, abandonment”, indicating that “Commission” contains the meaning of functional decline or not used. It can be seen that the ancient impotence can be generalized to refer to the human body tissues and organs withered, atrophied or functionally declined, or even abandoned a class of diseases, the scope of the impotence is much larger than today’s impotence disease. Tuo Hua Tuo secret medical biography of the first volume of “on the kidney deficiency, cold and heat, life, death and obedience to the law,” said “kidney? If the pulse is very rapid, then the impotence of the kidney is a disease in the obstruction of the intestine. This suggests that there is an ancient term for “impotence of the kidney”, but unfortunately, its clinical characteristics cannot be determined. Unfortunately, its clinical characteristics could not be determined, and no treatment measures could be found [1]. Combined with the modern medical viewpoint, we believe that “renal impotence” is a group of diseases in which the renal tissues wither, atrophy, or functionally deteriorate, or even become obsolete. It is also referred to as the modern renal excretory function is seriously impaired, resulting in nitrogen and other metabolic wastes stored in the body, at the same time, causing water, electrolyte and acid-base balance imbalance, the clinical body is characterized by its own toxicity of the critical syndrome, i.e. uremia. The main clinical manifestations are: loss of appetite, nausea, vomiting, fatigue, pallor, nocturia, long, frequent or unfavorable urination, swelling of limbs, skin itching, dizziness and headache. 2. The understanding of the pathogenesis of “renal impotence” Through years of research and evidence-based literature, we believe that: “renal impotence” is located in the kidneys, due to external and internal injuries caused by a variety of factors leading to damage to the kidneys, the kidneys, the kidneys, kidney opening and closing of the failure of the Department of the qi does not transform water, Yang does not transform turbidity, qi and blood running weakness, blood stagnation, water-dampness stagnation, water and dampness stagnation, the blood flow and stagnation. The blood flow is stagnant, and the water-dampness, phlegm, turbidity and stagnant toxins can not be drained outward and gathered in the body and accumulated in the internal organs, which becomes poisonous in the long run, damaging the spleen and kidneys, and depleting the organism and causing the onset of the disease. Although the late lesions can involve the heart, liver, lungs, spleen, kidney, five organs and the stomach, intestines, bladder and other organs, but always to the spleen and kidney qi deficiency as the key, and throughout the course of the disease. Clinical manifestations include the deficiency of the spleen and kidneys and the gradual decline of the main internal organs, and at the same time, it is often accompanied by dampness, turbidity, blood stasis, and toxicity of the real side of the evil. Forming a complex pathological process of mixed deficiency and reality, cold and heat. Danxi’s Essentials of Treatment? Edema”: “Because of the deficiency of the spleen can not line turbid qi, qi gathered for water, water impregnation delusion”. Su Wen? Hydrothermal point theory”: “Kidney, the stomach of the gate is also, close the door is not favorable, so the gathering of water from its class also, up and down overflowing in the skin, so for the swelling of the bowels. Visceral swelling, water and sick”. In short, “kidney impotence” is the basic pathology of kidney disease prolonged for a long time, the internal organs of the function of the deficiency, kidney qi decline, gasification power; spleen deficiency and loss of transport, water and grain. Blood lack of nourishment, blood stasis, dampness and toxicity congestion. Clear gas does not rise, turbid gas does not descend, dampness and toxicity stored in the body, not external solution, and further can be trapped in the pericardium, renal deficiency and wind movement, until the heart and kidney organs are defeated and end. 3. “Kidney impotence” principle of treatment According to Chinese medicine, “long illness is more virtual”, “long illness into the network”, “disease long more stasis According to the theories of Chinese medicine, “long-term illnesses are often weak”, “long-term illnesses into the network”, “long-term illnesses are often stasis”. According to the theories of Chinese medicine, “long-term illnesses have more deficiencies”, “long-term illnesses enter the network” and “long-term illnesses have more stasis”, the pathogenesis of “renal impotence” is based on the deficiencies of the spleen and kidneys, with dampness and stagnation of toxins as symptoms. According to the characteristics of the disease mechanism, we propose that the treatment principle should follow the law of supporting the positive and dispelling the evil, treating both the symptoms and the root cause. Depending on the patient’s specific condition, either the main focus is to support the positive, or the main focus is to dispel the evil. In the course of treatment, attention should be paid to supporting the positive and dispelling the evil without hurting the positive. Correctly handling the relationship between supporting the positive and dispelling the evil, and mastering the timing and preference of supporting the positive and dispelling the evil is the key to treating this disease, so it is emphasized to grasp the fundamentals of the spleen and kidney failure when treating the disease. Specifically can be used to strengthen the spleen and benefit the kidney, diuresis and dampness, drainage and elimination of toxins and blood circulation and dissipation of blood stasis therapy, so that the evil can be discharged from the body in a timely manner, in order to enable a certain degree of restoration of the wasted kidneys [3]. 4. Characteristics of the formula for treating renal impotence According to the characteristics of the pathogenesis of renal impotence and the principles of treatment, and based on our many years of clinical and basic research evidence of modern medicine [4-9], we have created a “renal impotence formula”. Based on our many years of clinical and basic research evidence [4-9], we created the “Kidney Impotence Formula” to treat “Kidney Impotence”.
