High intraocular pressure in patients with glaucoma is not a contraindication to cataract surgery. For patients with closed-angle glaucoma, cataract surgery is even more recommended to be done early. Only for patients with very high IOP, cataract surgery requires careful control of IOP fluctuations to prevent severe IOP fluctuations from leading to complications. Common complications include intraoperative or postoperative ciliary body choroidal detachment and acute suprachoroidal fluid leakage. There is even a risk of explosive suprachoroidal hemorrhage if combined with microphthalmia, hypertension, and advanced age. On the other hand, in patients with advanced glaucoma, the optic nerve is very sensitive to intraoperative IOP fluctuations, and some patients may not even be able to withstand the stress of surgery resulting in blindness. These need to be carefully evaluated preoperatively.  After adequate evaluation, cataract surgery can be performed for patients whose risk is manageable.
