How should melanoma be treated in its early stages?

  There are benign and malignant melanoma. Benign melanoma does not affect a person’s healthy life and can be cured, while malignant melanoma is rare, but not as malignant as it is difficult to cure. Early detection and treatment of any disease will greatly increase its cure rate, and melanoma is no exception, and can be cured if detected in time.  How should melanoma be treated in early stage?  1. Surgical excision of melanoma should be performed early. If the primary tumor is confirmed to be melanoma by frozen section, then the surgery should be done to expand the resection. The traditional theory is that the resection should include the 5 cm area outside the tumor. In recent years, however, there is a tendency to agree with Mohs that only 1 cm of the face should be excised and 2 cm of the rest of the melanoma should be sufficient. After 4-6 weeks of surgery, regional head and neck lymphatic drainage can be performed according to the direction of scalp lymph node drainage, which is helpful to improve the survival rate.  2.Physical therapy for melanoma is suitable for superficial and early lesions, using laser or liquid nitrogen, supplemented with radiation therapy after surgery.  3.Melanoma chemotherapy can slow down the deterioration of the disease for those who have metastasized.  (1) Anti-melanoma (triazolomide, DIC or DTIC), 2.5mg/kg per day, add 150ml, 5% glucose solution or saline for rapid intravenous infusion (10-15min), 10 days for a course of treatment, 3 weeks later, the second course can be done. It has inhibited bone marrow and gastrointestinal reactions and cold-like symptoms. For advanced patients, ipsilateral external carotid artery cannulation can be tried to facilitate multiple consecutive injections to improve the efficacy.  (2) BCNU, 2.5mg/kg per day, added to 250-500ml of glucose or saline intravenously, twice a week for 3 weeks.  (3) Lomustine (CCNU) 200mg orally once every 6 weeks, with metoclopramide injection before administration to prevent vomiting. Lomustine (CCNU) also has a bone marrow suppressive effect and requires repeated checks of white blood cell count.  4.Immunotherapy for melanoma should be done by intradermal injection of vaccine made from own tumor, once or twice a week.
