Erectile dysfunction (ED) is a common disease, according to the Massachusetts Male Aging Study (MMAS) statistics show that the prevalence of different degrees of ED in American men aged 40 to 70 years is 52% ± 1.3%. Recent data show that China’s population over 60 years of age has reached 132 million, and at a rate of 3.2% per year, a sharp increase in the age structure of China’s population has been transformed from adult to elderly. Because of the large population base in China, the absolute number of middle-aged and elderly men occupies the first place in the world, so the number of ED cases in China will be huge. The National Family Planning Commission estimates that there are about 80 million to 100 million patients with different degrees of ED in the country. Although PDE5 inhibitors (“Viagra”) have revolutionized the treatment of ED, they have limited efficacy in severe ED. After decades of research and development, the penile pacemaker (also known as a penile prosthesis or penile support), a high-tech medical product, has become the standard treatment for ED and is recognized as having the highest clinical success rate. It is suitable for patients with severe ED who have not been treated with drugs or other treatments and who are eager to restore their sexual function. A penile pacemaker consists of a pair of fluid-filled cylinders, a fluid pump valve to control penile erection, and a fluid capsule, which are connected by a connecting catheter, commonly known as a three-piece device. The pump valve is surgically placed in the scrotum, the capsule in the retropubic symphysis, and the cylinders in the white membrane cavity of the penile corpus cavernosum. It simulates the process of filling and relaxation of the penile corpus cavernosum, replacing the lost function of expansion and erection of the penis and allowing the patient to regain the ability to perform sexual acts. When sexual intercourse is required, the hydraulic pump in the scrotum is pressed and the fluid in the fluid sac is pressed into the cylinder to support the erection of the penis. After sexual intercourse is over, the fluid in the cylinder is returned to the fluid sac by pressing the control switch of the fluid pump valve, and the penis returns to a weak state. At present, this product is only available in the United States and China in the world. Since it is implanted into the white membrane cavity of the penile corpus cavernosum, the patient can also have ejaculation and orgasm during intercourse after the surgery, and it does not affect normal urinary function. There is also no difference in the appearance of the penis after surgery. Surgical implantation of a penile pacemaker for the treatment of ED is a very mature, well established and popular technique. Its advantages are: simple operation, ease of use, excellent concealment (not easily detectable even during a full-body physical examination); impromptu erection when there is passion or need, as in a normal person; 100% success rate during sexual intercourse; better satisfaction during sexual intercourse; no side effects of the material used. The main complications after penile pacemaker implantation are infection and mechanical failure, the incidence of infection is 2-5%; the incidence of mechanical failure is also very low, which is related to the type of pacemaker and surgical operation.
