How to use medication and diet for patients with high uric acid

It is currently considered that medication should be administered immediately for blood uric acid levels above 540 micromol/liter, and that medication is required for any combination of cardiovascular disease or abnormal glucose and lipid metabolism diagnosed as hyperuricemia. It is usually recommended to control the blood uric acid level within 360 micromol/liter (low level is needed to reach the standard in case of gout), and the medication should be continued for 6 months after reaching the standard. For the treatment of high uric acid, the following two types of medications should be chosen primarily in conjunction with dietary modifications. 1.Uric acid excretory drug: benzbromarone After taking it, uric acid in the blood will be excreted in large amount through urine, and then the uric acid concentration in urine will increase significantly. In order to prevent uric acid from crystallizing in the urinary tract, it is necessary to drink at least 1.5 – 2 liters (7 – 10 cups) of water daily and alkalize the urine with baking soda if necessary. 2.Inhibitors of uric acid synthesis: allopurinol, febuxostat Allopurinol is a classic drug to stop uric acid synthesis, but there are adverse reactions that induce serious rash. Therefore, it should be taken in small doses and gradually increased. It is recommended that a susceptibility gene test be performed before the use of the drug, and the use is prohibited for those with positive results. It is also necessary to drink a lot of water when taking it, and it should not be used in the acute phase of gout. Febuxostat is a new type of drug that inhibits uric acid synthesis. It is more effective than allopurinol at full doses and has fewer side effects, but there is a suspected risk of inducing cardiovascular adverse events. (1) Reduce the consumption of foods containing high purine, such as seafood, offal, and various kinds of alcohol. (2) Eat less sweets, beverages, and various vegetable soups. (3) Drink more water to promote uric acid excretion. (4) Eat more green leafy vegetables and fruits (5) Eggs and milk are low purine foods and can be used in normal amounts. Milk is beneficial, especially skimmed milk which does not need to be limited, but yogurt should be considered and only consumed in small amounts. (6) Promote healthy activities and active exercise.
