What is penile erectile dysfunction (ED)

Erectile dysfunction (ED) is one of the most common male sexual dysfunctions, and domestic and international medical statistics show that about 52% of men suffer from ED after the age of 40. Although ED itself is not life-threatening, it is closely related to men’s physical and mental health, and has a significant impact on the quality of life of both patients and their families. There are many causes of ED, generally divided into three main categories: psychological, organic, psychological and organic coexist. In the past, it was believed that psychological ED was the most common, but the current view is that more than half, or even up to 80%, of ED is organic or a mixture of psychological and organic causes. Normal penile erection is the result of hemodynamic changes in the penile corpus cavernosum produced under the combined regulation of neurological, endocrine, vascular and psychological effects. Therefore, abnormalities in any of the above can lead to ED. Common causes include: ① cardiovascular diseases such as arteriosclerosis, coronary heart disease, hypertension, ② endocrine and metabolic diseases such as diabetes, hyperlipidemia, hypogonadism, obesity, ③ chronic diseases such as chronic liver and kidney insufficiency, ④ penile sclerosis, penile curvature, prostatitis, prostatic hypertrophy and other genitourinary diseases, ⑤ Trauma or surgery such as spinal cord injury or surgery, pelvic fracture, urethral injury, pelvic surgery (rectal cancer, radical prostate cancer), 6) poor lifestyle such as long-term excessive alcohol consumption and smoking, 7) mental and psychological factors such as anxiety, depression, marital disagreement and lack of sexual knowledge. A reasonable diet, moderate exercise, smoking and alcohol cessation, and psychological adjustment are the basis for the treatment of ED, along with one or more treatment methods such as oral medication, negative pressure suction device to induce penile erection, and penile prosthesis (penile erectile device) implantation surgery, which can make the treatment efficiency reach more than 95%. ① In the history of mankind, probably no drug has been developed as enthusiastically as “Aphrodisiac”, but it was not until the 1990s that there was a breakthrough in the pharmacological treatment of ED. The three oral medications that have been marketed, Viagra, Cialis, and Elidel, have shown significant improvement in erectile dysfunction from a variety of causes and are recommended as the first-line treatment option for erectile dysfunction. ②Putting the penis into a plastic tube and using a negative pressure suction device to induce an erection is currently the second-line treatment for ED, with an erection efficiency of up to 90% and a satisfaction rate of 80% regardless of the cause of ED. ③ Most ED patients can prefer oral medication, and if oral medication fails or further treatment is desired, second-line treatment methods can be considered. For the above treatment failure or refusal of second-line treatment methods, penile prosthesis (penile erectile device) implantation surgery can be chosen to implant a prosthesis into the penile corpus cavernosum to achieve a penile circumference and hardness similar to a normal erection. Since the penile prosthesis conforms to physiological erection, is concealed in appearance, and does not affect daily life, penile sensation, urination and ejaculatory function, the satisfaction rate exceeds 90%, making it the most effective treatment for ED at present. With the development of medical technology, current treatment methods can completely cure most ED and significantly improve patient and spousal satisfaction, but each treatment method has its own side effects and strict indications, and requires strict guidance from professional doctors and competent large treatment centers for implementation. In contrast to the high prevalence of ED, effective treatment, the patient consultation rate is very low, they are ashamed to talk about it, or think that ED is related to age, not actively seeking treatment, and even more do not go to the regular hospital consultation, prefer some unreliable so-called “health products”, “secret recipes “ED is like high blood pressure, diabetes, is a common disease that most men may encounter in their lifetime. In addition, normal erectile function is a sign of male health, middle-aged men with ED can be considered to have a very high probability of diabetes, cardiovascular disease and other systemic diseases. Therefore, if suffering from ED or problems in sexual life, you should take courage, put down the burden of thought, timely diagnosis and treatment in regular hospitals, not only to treat ED itself, but also early detection or prevention of serious systemic diseases, thus helping to improve their own health and self-esteem, create a happy and fulfilling married life.
