Dry syndrome is not scary

In the past, dry syndrome was thought to be a rare disease, but in recent years, with the deepening of people’s understanding of rheumatic diseases and the progress of immune detection technology, the incidence of dry syndrome is second only to rheumatoid arthritis and even exceeds rheumatoid arthritis in China, and the trend is increasing every year. This disease is common in middle-aged women, with various clinical manifestations, varying severity, and easily complicated with multi-system damage, and is one of the difficult-to-treat diseases. Due to its characteristics of unknown cause, chronicity, recurrence and intractability, many studies and observations at home and abroad show that patients with dry syndrome generally have depression or anxiety, and the incidence is not only significantly higher than that of normal people, but also compared with patients with rheumatoid arthritis and lupus erythematosus, which are also rheumatic diseases, patients with dry syndrome also have more symptoms such as bad mood, irritability, headache, gastrointestinal symptoms and decreased concentration and memory. symptoms and decreased concentration and memory, etc. Some domestic scholars used zung-type depression and anxiety self-rating scale to evaluate the mood changes of 47 patients with dry syndrome, and the results showed that 23 of them had anxiety symptoms (49%) and 14 had depressive symptoms (30%), which were significantly different from the healthy group. The main manifestations were weakness (62%), depressed mood (50%), dyspepsia (40%), decreased sexual interest (36%), feelings of uselessness (31%), irritability (27%), and sleep disturbance (20%). This indicates that SS patients are more likely to exhibit anxiety and depression, which seriously affects the quality of life. In the clinic, we often see that once patients are diagnosed with this disease, they search for various knowledge and information about dry syndrome on the Internet and check with their own condition: or they are pessimistic and disappointed because they know that the disease cannot be cured; or they are worried because they hear that the disease is likely to cause multi-system damage; or they are afraid of the side effects of hormone or immunosuppressant treatment; or they are depressed because they are afraid of heredity to their offspring Or they may hear hearsay and use prescriptions indiscriminately; or they may see the slight fluctuation of the laboratory test after the review and have trouble sleeping and eating, and so on. This phenomenon is presumed to be caused by a variety of factors: Some studies have confirmed that the relative low function of the neuroendocrine axis (adrenal, gonadal and thyroid axis) due to the damage of the nervous system can partially explain the mood disorder of patients. In addition the immune system is disturbed in this disease, as evidenced by immune activation and cytokine release. There is evidence suggesting that certain cytokines may underlie anxiety and depression, and that elevated long-term cytokines (pro-inflammatory cytokines) may cause shifts in neuroendocrine and central nervous metabolites, among others, that appear or exacerbate anxiety and depression. The effects of somatic symptoms are more important. Dry mouth and eyes and joint pain are the main symptoms of dry syndrome. Due to the decrease of saliva and tear secretion caused by the lesion of exocrine glands, patients can show persistent and obvious dry mouth (incidence about 70-80%) and dry eyes, and those who lack saliva seriously need to drink water frequently and have difficulty in eating solid food, often complicated by dental caries, oral candidiasis and infections; lack of tears leads to the desire to cry and loss of vision, which can be complicated not only by The lack of tears leads to weeping and loss of vision, which can be complicated not only by intraocular infections but also by corneal ulcers, perforations or permanent blindness. These physical symptoms often cause many patients to suffer from mental distress and pain, gradually becoming anxious and depressed, and their quality of life declining. Psychosocial factors should not be ignored either. Dry syndrome is a chronic disease with a long course, and there is no cure yet, and it is easy to have recurrent attacks and multi-system damage. The side effects and adverse reactions of long-term hormone or immunosuppressive therapy are also important factors that increase the psychological burden of patients. The disease mostly occurs in women after 40 years of age, and the multiple factors such as family burden, economic pressure and hormone level changes make them prone to depression; the generation of depression is also closely related to the personality of different patients, and patients who are more introverted and not good at communicating with others are more prone to depression. The above situation suggests that doctors should not only achieve early diagnosis and standardized treatment of dry syndrome in the clinic, but also pay attention to the changes of patients’ mental and emotional aspects, pay attention to psychological guidance and mutual communication, so as to arouse patients’ positive emotions, correctly play the role of psychological defense mechanism, eliminate the psychosomatic pathological state and effectively improve the quality of life. For patients with dry syndrome, adopting a positive attitude and maintaining optimism are the prerequisites for overcoming the disease, enjoying health and striving for early recovery. First of all, we should have a correct understanding of the disease, including the following aspects: 1. The severity of dry syndrome varies. At present, it is believed that this disease is a heterogeneous disease, that is, due to the different reasons for the onset of the disease and the different genetic qualities of the patients, it is likely that the process of the pathogenesis is not exactly the same, thus the course, severity, type, treatment response and prognosis of the clinical manifestations are also different. In other words, some patients may have very mild disease and can be stabilized for life without hormone or immunosuppressant treatment, and can live, study, work and play like healthy people; of course, there are also patients whose disease starts with systemic damage of lung, liver and kidney, and need hormone, immunosuppressant and combined Chinese and Western medicine treatment to stabilize the disease and reduce the damage to the body. Therefore, there is a difference between the severity of dry syndrome and the early and late stage of the disease. Some patients often say what late stage of the disease, but the actual refers to the severity of the damage of internal organs. 2.How to judge the degree of disease. First of all, we should go to a regular rheumatology department of a large hospital for examination to confirm the diagnosis. Furthermore, it is important to clarify whether there is systemic damage and to determine its severity. Leukopenia is very common, and if it is around 3000, it will not cause serious consequences and general treatment is sufficient. Hormones can be used for thrombocytopenia. Severe interstitial lung lesions, liver damage and kidney lesions need specialist treatment. 3.To distinguish whether it is the symptom of dry syndrome itself or the symptom of anxiety and depression. As mentioned before, the anxiety and depression phenomenon of dry syndrome is more common. The common symptoms of dry syndrome itself are dryness of mouth and eyes, joint pain, repeated swelling and pain of parotid gland, multiple caries, repeated purpura-like rash of lower limbs, etc. If there is systemic damage, corresponding performance can be seen. Many symptoms such as depressed mood, nervousness and anxiety, anxiousness and irritability, insomnia, body discomfort, muscle aches and pains, and pains of irregular location and duration are mostly caused by anxiety and depression. Many patients can’t distinguish between them, and some symptoms are considered as disease activity, which aggravates the depression symptoms. 4.Is it better to treat dry syndrome with Western medicine or Chinese medicine? It needs to be treated differently according to the condition. If the disease is stable and there is no systemic damage, and the disease activity index is normal or slightly high, and the dryness of mouth and eyes and joint pain are obvious, it is possible to use Chinese medicine to improve the clinical symptoms. For those with active disease and no serious systemic damage, Chinese medicine alone or Western medicine alone can be used, or both Chinese and Western medicine can be used to control the disease in a stable state. For the treatment of patients with systemic damage, such as interstitial lung lesions, liver damage, vasculitis and pseudolymphoma, glucocorticoids and/or cytotoxic drugs are needed, while taking traditional Chinese medicine can enhance the efficacy through overall adjustment, reduce the toxicity and side effects of hormones and other western drugs, and delay the progress of the disease. 5.TCM treatment of anxiety and depression in dry syndrome. Dry syndrome belongs to the category of paralysis in Chinese medicine, and Chinese medicine believes that the disorder of emotion is one of the reasons for the occurrence of paralysis, and after the occurrence of paralysis, it is also easy to cause the change of emotion. As early as in Su Wen? In the “Treatise on Paralysis”, there is a record that “Yin Qi is the source of the static energy, but the agitation is the cause of death”, pointing out that the cause of paralysis of the five organs is related to the emotions. Luo Mei of the Qing dynasty said in the “Internal Classic”: “Where the seven emotions are used excessively, they can also injure the dirty qi and become paralysis.” And also said: “Liver paralysis, liver gas depression and blood does not glory tendons of the disease.” It can be seen that emotions cause paralysis is mainly related to the dysfunction of the liver, and the main treatment is to de-stress the liver and regulate the qi mechanism. The Chinese medicine treatment of anxiety and depression in dry syndrome can also be discussed from the theory of paralysis caused by emotion, and the treatment is based on the liver. Commonly used formulas include Chai Hu Dredging Liver San, Zhuan Yao San, Liu Wei Di Huang Wan, Gan Mai Da Zao Tang, Gui Shen Tang and so on, but they should be taken under the guidance of an experienced TCM practitioner. In a word, as long as you take it seriously, regular medical treatment and cooperation between doctors and patients, dry syndrome is not terrible and the condition can be controlled. I hope that patients with dry syndrome can read this article to increase some related knowledge, reduce the pressure of mind, overcome the disease and enjoy health.
