Degenerative osteoarthropathy, also known as osteoarthritis, degenerative arthritis, age-related arthritis, hypertrophic arthritis, is a degenerative disease caused by ageing, obesity, strain, trauma, congenital abnormalities of joints, joint deformities and other factors, including degenerative damage to joint cartilage, reactive hyperplasia of joint edges and subchondral bone. The disease is most common in the middle-aged and elderly population, and it occurs in weight-bearing joints and joints with high activity (e.g., cervical spine, lumbar spine, knee joints, hip joints, etc.). Excessive weight bearing or use of these joints can promote the development of degenerative changes. Clinical manifestations include slowly developing joint pain, pressure, stiffness, joint swelling, limitation of motion, and joint deformity. With aging, this type of patient is now very common. 1.Western medicine treatment At present, there is no effective treatment drug for this disease in Western medicine, often using symptomatic treatment, such as pain can take some antipyretic and analgesic drugs; numbness can be used for B vitamins; joint swelling with effusion can be given to local extraction of effusion or local closure and other therapies. However, these treatments are not ideal and the condition is prone to recurrence. Chondroprotective agents such as glucosamine sulfate can promote the synthesis of cartilage and inhibit the decomposition of articular cartilage, and also have anti-inflammatory effects. Sulfate itself is one of the necessary components for the synthesis of cartilage matrix. These drugs can relieve pain, improve joint function, and retard the destruction of joint structure over time (more than 2 years). Glucosamine sulfate has a slower onset of action, but has a good safety profile and is suitable for long-term use as a basic treatment. 2.Physiotherapy DC drug ionization method gives full play to the role of drugs, but because the imported drugs are liquid, easy to volatilize, the effect is not lasting, which limits the therapeutic effect of this method. The therapeutic effect of ultraviolet therapy is mainly in the following aspects: bactericidal effect; anti-inflammatory effect; analgesic effect; promotion of wound healing effect; desensitization effect; promotion of vitamin D3 formation; mediation of the body’s immune function, etc. Other methods such as infrared, microwave, laser and wax therapy can also be used with certain effect. 3.Surgical treatment Surgical treatment is not the first choice of treatment. When the conservative treatment is not effective and the condition is serious and seriously affects the patient’s life. Chinese medicine generally believes that the kidney is the master of bone, the liver is the master of tendons, when the kidney and liver qi is weak, it will affect the human tendons and bones, resulting in the weakening of the tendons and bones, coupled with external evil invasion of tendons and bones, it is easy to evolve into degenerative arthritis. The prescription consists of Duluxiang, Fructus Herba, Eucommia Ulmoides, Cinnamon, Poria, Niubizi, Hessian, Fenfeng, Chuanxiong, Ginseng, Licorice, Peony, Angelica, Gentiana, and Radix Rehmanniae. In the long-term TCM theoretical research and related clinical disease TCM pathology research, it is believed that degenerative bone and joint diseases, in addition to liver and kidney essence deficiency, wind, cold and dampness external evil internal offense tendons and bones, but also closely related to blood stasis internal knot, meridian pain, congenital life of the essence deficiency. Under the guidance of this understanding, the author formulated the formula of Nourishing Bone and Tongjing Tang ten years ago. The whole formula is guided by the combination of opening the meridians, resolving blood stasis, nourishing the liver and kidney, nourishing the vital essence, and dispelling wind and dampness, forming a formula composed of eight valuable Chinese herbs such as whole worm, Panax notoginseng, golden hair dog’s spine, Cistanches, and Gentiana. It usually relieves common symptoms such as numbness, headache, dizziness and stiffness significantly within a week. The author is even more pleased because by a chance, he found that the efficacy of this formula in bulk is more rapid and better than that of the original tonics. In ancient times, the dosage forms of traditional Chinese medicine were very complicated, such as ointment, elixir, pills, and powder, which were always prepared by doctors according to clinical needs. However, since the founding of China, due to various complicated reasons, hospitals rarely prescribe the dosage of pills, pills and poultices. But in fact, the efficacy of different herbal dosage forms may be very different, and deep down, this is the inevitable result of the different methods of preparation. The change in the dosage form of this formula causes the increase or decrease in efficacy. Moreover, this change from soup to powder also significantly enhances its future commercial development value, making it possible to benefit more patients with degenerative bone disease.
