Li, male, 41 years old, suffered from acute myocardial infarction in October 2012 and was treated conservatively in a local hospital in Shandong. Due to decreased activity endurance, post-activity wheezing, and inability to lie down after wheezing in March 2013, he visited Beijing Fuwai Hospital. After investigation, the coronary angiography results showed: left main stem + three branches lesion, which was not suitable for stenting and bypass surgery due to severe stenosis with diffuse lesions. Cardiac ultrasound examination: EF 26%. The patient’s poor cardiac function led to shortness of breath, wheezing, episodes with slight activity, and inability to take care of himself. Fu Wai Hospital recommended the patient to undergo heart transplantation. Since he had to wait for a heart donor source, he went to Dongzhimen Hospital outpatient clinic. After detailed examination of the patient, medical history and examination data, we considered that the patient had old myocardial infarction and low cardiac function, which caused the patient’s reduced mobility, wheezing and suffocating with slight activity, and usually unable to take care of himself. There is chest tightness and a dark tongue after activity, because Qi deficiency cannot promote normal blood flow, resulting in blood stasis and poor blood flow. The patient has no obvious edema in the lower extremities, but low urine volume and mild edema of the head and face, which is considered to be a deficiency of kidney qi, unable to qi-transform water and fluid and stagnation in the body.
