Cryptorchidism is a common disease in children, which manifests itself as a testicle in the scrotum after birth, either unilaterally or bilaterally, and can be temporarily observed in children under 2 years of age. If the testicle does not descend to the scrotum after 2 years of age, surgical investigation and testicular descent fixation should be performed. Otherwise, it may affect the normal development of the testis and cause malignant changes. There are two types of surgery: traditional surgery and laparoscopic surgery, the purpose of which is to investigate whether the affected testicle is developing normally and to fix it in the scrotum by traction (testicular descent fixation). Our experience is that if the testicle is felt at or below the inguinal canal during the preoperative examination (lower position), traditional surgery can be chosen (lower cost); if the testicle is not felt before surgery (higher position), trans-laparoscopic exploration should be used, and if a normal testicle is found, laparoscopic testicular descent fixation should be performed (less invasive). The advantages of laparoscopic surgery for children whose testicles cannot be felt before surgery are: 1. Avoid the large incision of traditional surgery. Since high cryptorchidism can theoretically reach the level of kidney, if testis or spermatic cord and vas deferens cannot be found, it should be explored upward to the level of kidney, and a big incision must be made to complete it. In contrast, laparoscopic surgery can be completed with only 3 incisions of less than 1cm in the abdomen. 2, laparoscopy can explore the ectopic testis to avoid possible leakage during conventional surgery. Adult surgery has found that some patients with ectopic testicular malignancy have had conventional testicular exploration, but no testicles were found. This is because some of the testes may be ectopic to the opposite side and cannot be explored by conventional surgery. And this can be avoided with laparoscopic surgery. 3.Children with bilateral cryptorchidism can be operated at the same time without additional incisions. 4.It can explore whether there are other diseases associated with it and treat it at the same time if necessary. Therefore, we suggest that laparoscopic surgery should be performed in all cases of high cryptorchidism.
