Sudden deafness is one of the common acute diseases in otorhinolaryngology, and its incidence has been increasing year by year in recent years. Most of the patients’ incidence is closely related to mental stress, negative life events and other adverse emotions. This paper intends to briefly analyze the relevance of internal injury of emotion and will to provide useful ideas for the treatment of sudden deafness by combining Chinese medicine theory with modern research. 1. Internal injury of emotion and will firstly injures the liver. Emotional and intellectual injury is one of the etiological theories in Chinese medicine. According to Chinese medicine, emotional and mental activities are closely related to the physiological functions of the five organs, especially to the liver, and it is said that “the liver is the master of emotions”. Because the liver is the general’s official, the liver collects blood, is the master of drainage, likes to regulate, and hates depression, the body yin with yang. The normal emotional activity mainly depends on the normal operation of qi and blood. If anger hurts the liver, liver fire, or liver qi is not comfortable, emotional and mental disorders, can cause the liver body to lose flexibility, liver qi stagnation, qi and blood run erratically, resulting in systemic diseases. 2.1 The physiological function of the ear is closely related to that of the liver. The liver collects blood, and the functions of all organs in the human body require the regulation of blood by the liver, as stated in “Su Wen – Five Viscera Generation”: “The liver receives blood and can see, the foot receives blood and can walk…” and so on. Hearing is the main physiological function of the ear, which also requires the supply and moistening of liver blood to complete, and the ear can hear only when it receives blood. If liver blood is deficient, the ear receives insufficient blood, and if blood is deficient and yin is lost, the ear orifice loses nourishment, then hearing loss, tinnitus and vertigo will occur. The draining function of the liver also plays a coordinating role in maintaining hearing acuity. If the draining function of the liver is normal, the qi flow will be well organized, the ear orifices will be open, and hearing will be acuity. At the same time, the liver is the master of draining and has the physiological characteristic of being the master of wind initiative. Therefore, the draining and active characteristics of the liver are closely related to the ear’s ability to help the body balance. If the liver loses drainage, the qi does not flow smoothly, or if liver depression turns into fire and liver fire disturbs, it can lead to tinnitus, deafness, vertigo and other z-syndromes. 2.2 The liver and gallbladder meridians are closely related to the ear: the gallbladder is the internal organs of the central essence, the official of the central right, the biliary organs of Shaoyang enter the ear from behind the ear, go out in front of the ear, and travel to the complete bone behind the ear, as said in the chapter “Ling Shu – meridians”: “The pulse of the gallbladder foot Shaoyang starts from the eye canthus, goes up to the corner of the head, goes down behind the ear, …, its branches, from behind the ear into the ear, go out in front of the ear, to the eye canthus After.” Therefore, if the bile qi is smoothly flowing, and the inner organs of the bile are uncluttered, and the essence is transferred upward, then the ear veins are smooth, the ear orifices are clear, and the body is not tilted. The liver and the bile are in close proximity to each other, and the liver meridians are also closely related to the ear orifices. Therefore, it is said that “the ear disease is actualized by Shaoyang”. With the accelerated pace of modern life and increased social pressure, the incidence of sudden deafness tends to rise gradually, and the incidence of deafness is mostly related to bad emotions. The liver is the official of the general, which is responsible for the promotion and draining of the liver. If anger hurts the liver, liver qi becomes depressed and reverses, blocking the clear orifice, or if depression causes depression, liver fire hurts yin and cannot control yang, which causes hyperactivity of liver yang, and the extreme of hyperactivity of yang leads to internal movement of liver wind, so symptoms such as violent deafness, tinnitus and dizziness appear. Although the influence of liver and gallbladder on the ear orifice is multifaceted, multi-linked and multi-channel, its basic pathological changes are the disturbance of liver fire, stasis of blood and loss of nourishment of the ear orifice. The modern pathological mechanism that causes sudden deafness may be due to the sudden change of patient’s emotion, which causes the change of neurohumoral regulation, blood stasis and increase the physiological response of the organism to biochemical stimulation, resulting in the dysregulation of sympathetic nervous system activity, leading to insufficient blood supply to the inner ear, causing tissue edema, hypoxia and metabolic disorders, and damage to the peripheral receptors of the inner ear. 4. There is a certain similarity between the pathological mechanism of emotional and psychological stress. Psychological stress is one of the pathological processes in modern medicine. It is believed that the liver is one of the innate determinants of psychological stress. The liver regulates the stress response of the body through its influence on qi, blood, fluids and emotions. The liver’s function of draining blood can maintain and promote the normal operation of qi and blood throughout the body and regulate the response of each organ to stress; the liver’s function of collecting blood and regulating blood volume can improve microcirculation and promote the redistribution of blood throughout the body in stressful conditions. According to Zhou Ping et al, there is a great similarity between Chinese medicine’s emotion-causing diseases and modern psychological stress theory in terms of understanding methods. The liver in the will is anger, and negative life events are the initiating factors of anger-induced diseases, so there is a certain similarity between anger-induced diseases and psychological stress. 5. Prevention and treatment of sudden deafness emphasizes the regulation of emotion and will, and the treatment from liver and gallbladder is effective. Deafness caused by liver and gallbladder is either due to liver qi stagnation, stagnation of the ear orifice, depression and fire, or due to violent anger injury to liver, liver fire inflammation, loss of clarity in the ear orifice, or due to liver fire injury to yin, yin does not control yang, liver yang is hyperactive. Patients often have tinnitus or thunder like wind and thunder, with sudden aggravation of symptoms after anger, hearing loss, red face and eyes, headache, dizziness, restless sleep at night, heart trouble and other symptoms. Therefore, the prevention and treatment of this disease should pay attention to the regulation of emotion and will, in parallel with drug treatment, with psychological guidance; treatment should be to clear the liver and Qi, blood circulation and orifices. In our clinical practice for many years, we pay attention to the relationship between emotional and mental factors and treat sudden deafness from liver theory. We have formulated our own formula to clear liver and clear orifices (Chai Hu, Gentian Cao, Hooked vine, Scutellaria, Gardenia, White peony, Shengdi, Sour date palm, Yuan Zhi, Dan Shen, Ge Ge Ge, Calamus, Chuanxiong, etc.), which has been clinically applied for many years with remarkable efficacy. The pathogenesis of sudden deafness caused by emotional factors needs to be further investigated in order to provide a reliable theoretical basis and effective treatment for the prevention and treatment of sudden deafness.
