The pharmacology of Leigongjiang and its clinical application

  Radix Polygoni (alias: Radix Polygoni Tablets, Radix Total Glucoside) is an extract from the root core of Radix Rehmanniae, a plant of the family Weimaraceae, which has various pharmacological activities such as anti-inflammatory, immunosuppressive, anti-tumor, reducing proteinuria and affecting reproduction, and has unique efficacy in rheumatoid diseases, renal diseases, skin diseases and other autoimmune diseases. However, its clinical application is limited by its high toxicity, which can be accompanied by adverse effects such as nausea, loss of appetite, leukopenia and thrombocytopenia, menstrual disorders, and sperm reduction. In order to make Leigengtang polysaccharide better used in the clinic, its toxic side effects are reviewed as follows.  1.Digestive system damage The most common side effects of Radix Polygoni in the digestive system are: nausea, abdominal pain, diarrhea and damage of liver function.  2, Reproductive endocrine system damage Women mainly show reduced menstruation and amenorrhea; for men, the side effects of this drug are mainly in the reduction of sperm, leading to infertility. This effect is reversible, and most of them can return to normal after stopping the drug.  3, blood system damage mainly manifested as red blood cells, white blood cells and platelet reduction, generally can be restored after discontinuation of the drug.  4, skin mucous membrane damage, skin pigmentation.  5.Renal damage High doses of Radix Polygoni are associated with renal failure.  The application of Leigengtang polysaccharide in the field of kidney Leigengtang has good therapeutic effect on some common glomerular diseases, including primary glomerulonephritis and nephrotic syndrome type I, allergic purpura nephritis and lupus nephritis. It is more effective in those with microscopic pathological changes and less effective in other types. It is more effective for those with proteinuria and hematuria as the main manifestation, but less effective for those with hypertension and renal hypofunction.  The efficacy of Radix et Rhizoma on IgA nephropathy is closely related to the clinical manifestations and pathological changes. The diterpenoids contained in Radix et Rhizoma have been shown to be a novel immune agent with similar effects to adrenocorticotropic hormones, but by a different mechanism and without the side effects of hormones. The effect on interleukin-2 is similar to that of cyclosporine, but the point of action is completely different. An important feature that makes it superior to other immunosuppressive drugs is that it has a significant selective effect on T cells, and its immunosuppression is focused on activated T cells. This new immunosuppressive drug is effective not only in certain types of chronic nephritis, but also in lupus nephritis, purpura nephritis and autoimmune disorders. In recent years, it has been shown to have a significant inhibitory effect on organ transplant rejection, and has a wide range of promising clinical applications that have attracted the attention of the international medical community.
