What should I do if I have blue patches of DVT treated with Warfarin?

Patient: Hello: I am a doctor, my loved one has formed left common iliac, left external iliac, deep femoral and superficial femoral vein thrombosis for 2.5 months after meniscus removal. She started with 7 days of low molecular heparin, then took Warfarin for a long time, and changed to imported Warfarin 1 week ago. INR 1.5~2.0, and is still adjusting the dose. Three days ago, I found a blue patch of 0.6*0.6cm under the toenails of both feet symmetrically, which looks like purpura. The blue patches under the toenails of both toes are symmetrical 0.6*0.6cm in size, what could they be? Li Youshan, Department of Peripheral Vascular Medicine, Beijing Dongzhimen Hospital: 1. Consider subcutaneous bleeding, but generally the location of bleeding is in the mucosal layer. 2, clarify whether there is fresh thrombus, if so, timely thrombolytic treatment is needed, anticoagulation alone cannot be relied on; if it is an old thrombus, ultrasonic ablation is needed if necessary. 3.To clarify the blood condition, thromboelastography is needed. 4.All the above cases need to be clarified as soon as possible. 5.It is recommended to stop Warfarin and change to low molecular heparin.
