Rhinoplasty is a very popular cosmetic procedure, especially popular with summer students. Many people’s understanding of rhinoplasty is limited to the improvement of the bridge and tip of the nose, but in fact, rhinoplasty is also a very critical part of rhinoplasty. If the importance of the root of the nose is ignored before and during surgery, the results of rhinoplasty surgery will be greatly reduced. Today, most rhinoplasty surgeons still do not pay much attention to the root of the nose, why is this? There are three possible reasons: first, they do not realize that the nasal root area, especially the nasal root point, is an important structure that determines the attractiveness of the nose from an aesthetic point of view and in terms of its composition and proportions; second, they do not know how to analyze the nasal root and determine its correct position and height; third, they cannot increase or decrease the height of the nasal root as they wish. Aesthetics: the nasal root point affects the entire appearance of the nose When analyzing the nasal root area, the nasal root point and the nasal root area must be strictly defined. In simple terms, the nasal root point is the deepest point of the nasofrontal angle, while the nasal root zone is an area (E) centered on the nasal root point, down to the level of the external canthus and up to the same distance. The nasofrontal angle organically connects the two concepts of nasal root point and nasal root zone. Usually, the nasofrontal angle is the main aesthetic factor affecting the lateral view of the nose, and the position and height of the nasal root point is another equally important aesthetic factor. The nasal root point is mostly located between the eyelashes and the upper crease (F). The height of the nasal root point, also known as its prominence, can be obtained by measuring the distance between the vertical tangent line to the seal or the cornea (G). These two parameters determine the position of the nasal root point, which in turn determines the starting point of the nose, which in turn determines the size of the nasal facial angle and the length of the nose. From an aesthetic point of view, the root of the nose affects the overall appearance of the nose; the lower the dorsum of the nose, the more bulky the nose looks, and the higher the dorsum of the nose, the smaller the base of the nose looks. Therefore, determining the location of the nasal root point is usually the first step in rhinoplasty surgery. Surgical plan: Rhinoplasty needs to take into account all aspects of the nasal dorsum and the seal Marking the nasal root point and its level and height. The root point can be positioned at the level of the eyelashes to the upper lid fold if a firm nose is desired, or at the level of the pupil if a soft nose is desired. The ideal height is 4-6 mm behind the seal or equal to the corneal tangent line. After confirming the position of the nasal root point, draw a vertical line through the root point and measure the size of the nasofacial angle, which can be set to 34 degrees for girls and 36 degrees for men. This will determine how much alteration should be made to the dorsum of the nose to facilitate proper correction. If there is a concern, do not make large changes to the nasal root (any change greater than 4mm in all directions is unrealistic). If the nasal root area is altered, the root and the dorsum of the nose must be clearly distinguished. Dividing these two areas with an external canthus line can be helpful in developing a surgical plan. Usually a two-step approach is used to determine whether the nasal root should be raised or lowered after determining the height and level of the nasal root point. Raising the nasal root usually reduces the degree of dorsal reduction.
