What is nerve block therapy?

In addition to the use of non-steroidal anti-inflammatory drugs, opioids, and analgesic aids, the nerve block technique is arguably the most common technique used by pain physicians. It is effective for many tendon sheaths, bursae, tendonitis, sacroiliac arthritis, pear-shaped muscle syndrome, tennis elbow, and carpal tunnel syndrome in the shoulder, neck, low back, or extremity areas. In postherpetic pain, trigeminal neuralgia, sciatica, phantom limb pain, burning-like pain and other refractory neuronal pain, the application of specific nerve block technique and nerve frequency (thermal) electrical modulation stimulation has achieved satisfactory therapeutic effects by blocking nociceptive conduction pathways, improving nerve trophic status and adjusting nerve conduction function. In actual clinical treatment, some diseases are ineffective with conservative treatments such as drugs, and are not suitable or cannot tolerate surgical treatment. If nerve block therapy is used, the effect that cannot be obtained by drug therapy can be obtained, and it is less damaging, faster and more tolerable than surgery for patients. In other words, nerve block therapy is a treatment measure between conservative drug treatment and surgical treatment. Some people call nerve block technique closed therapy, which is wrong. The term “closed” was created in the 1950s when the understanding of pain was shallow and the treatment means were few. It is understandable that the translation was wrongly done in Russian. Who knows, the mistake became a habit, and now many doctors still consider the treatment done by pain doctors as closed therapy. The term “closure” is widely used in China, is it the same as nerve block? Which one is the correct medical term? Nerve block” is a term that has been used worldwide for nearly 100 years and is still in use today. Nerve block refers to a treatment method that blocks the transmission of nerve impulses by means of drugs, and should not be translated as “injection closure” or “closure therapy”. At present, some doctors who are not specialized in pain management, and even some nursing staff, due to the lack of systematic knowledge and training, are enthusiastic about injecting various drugs at pain points, acupuncture points or nerve endings to relieve pain, and they call themselves “closed therapy”. Some add small amounts of local anesthetic drugs to the injection solution, while others do not. Some use a wide variety of drugs for local injection, including antibiotics, animal blood products, animal organ products, Chinese medicine, anti-immune agents, anti-tumor drugs and so on. Although this type of treatment can sometimes relieve the pain of some patients, many instances of complications and medical disputes have occurred. There are also some social loafers, to make money for the purpose of “a needle away”, everywhere “closed, so that the word “closed” adds a bad color. This “closed therapy” name is widely circulated, so that some medical personnel and some patients on the regular “nerve block” also have misunderstanding, and even have fear. It is not appropriate for a regular doctor to use the informal term “closed”. Furthermore, the term “closure” is not found in the English, Russian, or Japanese pain literature, and is better referred to as nerve block therapy in pain management, which is a scientific and standardized term used worldwide. The effect achieved by nerve block therapy is medically based on at least three aspects: first, blocking sympathetic nerves, causing vasodilation, edema reduction, pain relief and relief of sympathetic tension due to the combination of the disease; second, blocking sensory nerves, blocking the conduction of pain and inhibiting symptoms induced by sensory nerve stimulation; third, blocking motor nerves, causing muscle relaxation or temporary braking, so that the painful area gets “rest”. Nerve block therapy is a treatment method that uses local anesthetic drugs such as lidocaine or bupivacaine, together with steroids and vitamins injected into the site of nerve pain to eliminate inflammation and relieve pain. However, many patients do not know much about nerve block therapy and often do not easily accept it, thinking that the injected drugs contain hormones, which are not good for the body and can cause osteoporosis, etc. Even some orthopedic surgeons may oppose it. In fact, it is far from necessary to be so nervous about nerve block therapy. If we understand its effects, indications and contraindications, we will understand and accept it. For nerve block therapy to achieve the desired effect of treatment, three important components must be passed, namely, correct diagnosis, effective drug composition, and superb injection technique. Local anesthetic drugs such as lidocaine or bupivacaine in nerve block therapy are anesthetics with affinity for the nervous system, which can block the vicious cycle of pain and enable the nervous system to rest and adjust, and can protect the nervous system. It in turn has a strong analgesic effect and can block the pain signals from local lesions. Most soft tissue pain is mostly due to local aseptic inflammation and soft tissue congestion and edema that irritate the nervous system. Anesthetic drugs such as lidocaine combined with steroids (hormones) can alter local blood circulation, reduce inflammatory exudation, and promote the discharge of local metabolites, thus eliminating local inflammatory edema, promoting inflammatory absorption, and relieving muscle spasm. For chronic lesions with adhesions, add drugs that activate blood circulation, resolve blood stasis, regulate qi and relieve pain, in order to eliminate adhesions, soften hardness and disperse knots, remove stasis and create new ones, improve tissue metabolism and relieve pain with various effects. Most cancer patients require additional hormone therapy, especially for pain and numbness in the lower limbs caused by rapid spread and metastasis of cancer cells to the vertebrae. Some people worry that the hormonal drugs used may cause side effects to human body, but in fact, because the amount of drugs used in nerve block therapy is small, localized and not for long-term use, it will not bring any effect to human body. Therefore, nerve block therapy has the advantages of direct drug delivery and rapid therapeutic effect. Some diseases can be cured after treatment. The treatment should be clearly diagnosed, aseptic operation, accurate site, and appropriate medication. Once the nerve block therapy is chosen, the treatment should be adhered to and completed, so as not to stop in the middle of the treatment course and not to achieve the desired effect. If the nerve block therapy is operated properly, there is basically no danger, no special discomfort to the patient, the amount of locally applied steroid drugs is small, slow absorption, long maintenance time, and no obvious side effects to the body. However, any drug has certain side effects, and nerve block therapy may also produce certain dangers, such as local acute allergic reaction, local infection caused by improper aseptic operation, etc. Since the application of hormones reduces the local immune capacity, if unfavorable infection control occurs, local tissue necrosis, abscess and sinus tract formation may be produced. The use of hormones in nerve blocks to enhance the therapeutic effect has a history of nearly 60 years. Of course, the principles of caution and responsibility, rational application, on-demand use, short-term and appropriate dosage, and prevention of abuse should be followed in the application of hormones.
