High blood pressure, don’t rush to take medicine first

  With the rise of the aging population, the incidence of hypertension is also gradually rising, hypertension may seem like a simple disease of the elderly, but there is a lot to learn from it. As we all know, “hypertension” so far there is no cure, once the diagnosis of hypertension, the need to take long-term antihypertensive drugs to prevent further heart, brain and kidney damage, such as coronary heart disease, cerebral infarction, cerebral hemorrhage, kidney disease, etc., where the hypertension refers to “primary hypertension This refers to “primary hypertension”. However, some of the hypertension may be cured by other methods without medication, we call it “secondary hypertension”, that is, the symptoms of elevated blood pressure caused by other diseases or causes.  What kind of hypertension should we consider as secondary?  The most common ones are the following: 1. As seen in the case, the blood pressure is mildly or moderately elevated, with episodes of muscle weakness, or with symptoms such as polyuria and irritability. It may be caused by an increase in the secretion of a substance (aldosterone) that raises blood pressure due to adrenal cortical hyperplasia or adenoma. If surgery is not possible, the choice of medication requires the selection of a specific drug that has an antagonistic effect on aldosterone: spironolactone.  2. Sudden type. This type of person may usually have a relatively stable blood pressure, but may have a sudden onset of blood pressure increase due to emotional excitement, position change, urination, defecation, trauma and other bending triggers, which may exceed 200/100mmHg, accompanied by tachycardia, headache, sweating, pale or red face, which can be relieved in a few minutes or 1-2 hours, and may be “pheochromocytoma “. This kind of hypertension is like a button in the body, which is accidentally touched, and an alarm will be sounded and a sudden increase in blood pressure will occur. This tumor is mostly benign, and checking blood or urine catechol levels can help diagnose it. This tumor can grow everywhere in the body, but is more common in the adrenal glands, and can be localized by CT. Blood pressure can be normalized after surgical removal 3. Accelerated type. If the original blood pressure is not very high, but the recent rapid progress or sudden aggravation, taking “xxpril” or “xxsartan” antihypertensive drugs is ineffective, we should suspect whether there is bilateral renal artery stenosis. Renal artery stenosis causes ischemia in the kidneys, and the kidneys want to counteract this ischemia by releasing a hormone that raises blood pressure in the hope that more blood flow will be available. This disease can be diagnosed with a renal artery ultrasound or imaging, and if diagnosed can be eliminated by placing a stent in the narrowed renal artery.
