A. Intraductal papilloma is a type of benign breast disease that can easily become cancerous and is usually classified as a precancerous lesion. It is usually classified as a precancerous lesion. People who have this disease do not need to be nervous all the time, but it is necessary to pay attention to it. We will briefly introduce the follow-up treatment and treatment methods. In the case of intraductal papilloma or intraductal papillomatosis, total mastectomy + stage I mastectomy should be considered in one of the following cases (1) If there are female relatives with breast cancer in the family, then BRCA-1 and BRCA2 gene tests can be considered. If the test results have gene mutations or defects, then it is considered that there is a genetic factor that makes it more likely to become cancerous in the future. In this case, total bilateral mammary gland excision can be considered (but the nipple-areola complex can be considered to be preserved) and implant molding can be placed in stage I. (2) There are no mutations in BRCA-1 and BRCA2. However, those who still have multiple similar nodules in the breast on ultrasound or breast MRI may be considered for total subcutaneous adenomectomy with preservation of the nipple-areola complex (same method as described previously). (3) Pathologically confirmed intraductal papilloma combined with atypical hyperplasia, especially severe atypical hyperplasia, or those who already have cancer, can be treated by the same surgical method described earlier, or even simple mastectomy. Except for the above three cases, mastectomy (including total subcutaneous adenomectomy) is not recommended. In the case of intraductal papilloma or intraductal papillomatosis without “second” description, follow-up treatment to prevent its carcinogenesis should be performed. The main treatment is endocrine therapy: (1) endocrine drug toremifene, 40mg/day, 7 days after menstruation, 7-14 days in a row, and so on for the next month, for a total of 2 years. (2) You can also choose tamoxifen tablets, 20mg/day, and start the medication after 7 days of clean menstruation every month for 5-10 days, and so on for the next month, for a total of 2 years of adherence. (3) Choose any one of the above drugs, plus a kind of anti-breast enlargement Chinese medicine with treatment, such as Yanlu Breast Health Tablets, Breast Dispersing Capsules, Red Gold Anti-Knot Capsules, etc. Take the medicine starting 7 days after menstrual cleansing, and use it for 10-14 days continuously, stop taking it during menstruation, and so on in the next month. (4) The main common side effects of toremifene and tamoxifen are: it may lead to menstrual disorders, very rarely hair loss and decrease in white blood cells, etc. It is rare to cause endometrial thickening because the dosing time is different from that of breast cancer patients. Suggestions: Regular blood tests for routine blood and liver function at the hospital (can be checked after 2 months at the beginning, and if normal, can be rechecked once every 6 months). (5) If the side effects are obvious then you need to stop the drug, after stopping all can recover. (6) Menopausal women with good economic condition can also choose exemestane tablets for cancer prevention treatment. Fourth, after surgery for intraductal papilloma or intraductal papillomatosis, regular checkups are needed: (1) Ultrasound and physical examination of the mammary glands once every six months. (2) If you are older than 40 years old, you can have a mammogram once a year. (3) Magnetic resonance imaging of the breast if necessary. (4) If the condition worsens, surgery should be performed as soon as possible.
