Early stages of facial palsy should be treated with acupuncture as early as possible!

Facial palsy is a common disease in acupuncture, and because it affects the appearance and is a facial problem, it is very psychologically stressful for patients, especially young patients. It also causes a lot of disturbance to one’s daily life because it affects eating and drinking. Today, we introduce some common questions about facial palsy, hoping that patients will unload their baggage, look at the disease correctly and cooperate with doctors to achieve the best results. First of all, there are two main types of peripheral facial palsy from a pathological point of view. One is considered to be caused by immune inflammation of the facial nerve, and the diagnosis is called idiopathic facial neuritis, also known as Bell’s palsy; the other is caused by viral invasion of the facial nerve, which can be accompanied by herpes around the ear, and the diagnosis is called Hunt’s syndrome. The incidence of idiopathic facial neuritis is higher than that of Hunt’s syndrome. In comparison, the former has a somewhat better treatment outcome and a more optimistic prognosis. Therefore, when symptoms of facial palsy appear, first look at the auricle or behind the ear to see if there is any herpes, so that you know the cause of the disease. Second, idiopathic facial neuritis is a disease with a high rate of self-healing. According to a clinical epidemiological study abroad (Denmark), 70% of patients with idiopathic facial neuritis can heal on their own. Therefore, patients with idiopathic facial neuritis should have full confidence in overcoming the disease. In the clinic, we often see patients with facial palsy who are very anxious about not being cured. We would like to say here that most of the facial palsy can be cured by actively cooperating with doctors and starting regular treatment as early as possible. The timing of treatment is very critical. If treatment is started within the first week of onset and the symptoms are relieved, the cure rate of such patients can reach 94%, which can be said to be quite high. Third, the problem of hormone therapy. Doctors often give patients oral hormone therapy during their visits. So, why does facial palsy need to be treated with hormones and what kind of patients are suitable for hormone therapy? First of all, we should clarify that the facial palsy mentioned here is due to peripheral facial palsy caused by immune inflammation or viral infection, and facial palsy caused by intracranial lesions is not included. In principle, both types of peripheral facial palsy can be treated with oral glucocorticoids. This is because after the facial nerve originates from the brainstem, it has to pass through a bony canal called the foramen caudalis before it can come out of the skull and distribute to the facial muscles. Inflammation of the facial nerve will cause edema of the nerve, at which time the hard bony canal will compress the nerve, resulting in loss of innervation of the facial muscles and symptoms of facial palsy. If hormones are used appropriately in the early stage of facial palsy, the nerve edema can be effectively suppressed, which is beneficial to the functional recovery of the facial nerve. Therefore, early, full course and regular use of hormones is the key to obtain the effect of facial palsy in the early stage. Usually, physicians will decide the dosage of hormones according to the patient’s condition, age, weight and underlying disease. In my outpatient practice, I usually give oral prednisone to patients with idiopathic facial neuritis or Hunt’s syndrome, along with oral calcium and gastric mucosal protectors to prevent adverse effects of hormones. Hormones are not indicated for every patient. The elderly, patients with high blood pressure, diabetes, tumors, severe osteophytes, patients with tuberculosis and patients with uncontrolled infections should be very cautious and some are prohibited. Therefore, patients should never take hormones on their own. Finally, I would like to introduce the timing of acupuncture treatment intervention. Patients often go to a western hospital after developing facial palsy, and the doctor will often instruct the patient to wait for a week after the onset of the disease before proceeding with acupuncture treatment while giving western medicine. In fact, any disease should be diagnosed and treated as early as possible. As a traditional therapy, acupuncture has a positive effect on the recovery of facial palsy symptoms. Especially in the early stage of facial palsy, appropriate acupuncture techniques, together with moxibustion treatment, can get rid of external evil and restore nerve function as early as possible. The main reason why Western doctors do not recommend early acupuncture is because they are afraid that premature acupuncture stimulation may cause facial muscle spasm, but up to now, no study at home or abroad has confirmed such adverse effects of acupuncture. The main reason for facial muscle spasm in some patients with facial palsy is the slow recovery process of nerve function, during which the nerve grows abnormally, causing spasm or linkage. This is a sign of poor prognosis and has nothing to do with whether or not acupuncture is used in the early stages of treatment. The clinical studies we have done have confirmed that acupuncture can shorten the course of facial palsy patients, allowing for earlier onset and healing compared to the use of basic Western medicine treatment alone. In this case, the chances of sequelae symptoms such as muscle spasms will of course be reduced. Therefore, as acupuncturists we do not exclude the use of western medical treatments such as hormones and antiviral drugs. At the same time, we encourage patients to have an objective understanding of the problem of facial palsy and not to be fooled or frightened into it. The greatest advantage of acupuncture is to free patients from facial palsy as soon as possible, as well as to help heal refractory cases where healing would otherwise be unlikely. For facial palsy, acupuncture is a safe, highly effective treatment.
