There are many patients and even physicians outside of general surgery who struggle with the choice of laparoscopy for various procedures, such as the choice of surgical approach for hernia repair, which does seem to be a problem. We believe that there are advantages and disadvantages to using either open or laparoscopic hernia repair, and that each of these surgical approaches has its own relative indications: 1. Laparoscopic hernia repair has relatively small and discrete incisions, resulting in less trauma, less pain, and faster recent recovery. However, there are three small incisions (no additional incisions are made when performing bilateral hernia repair) and they are in relatively exposed locations; basically, general anesthesia is adopted, with the corresponding risk of general anesthesia; the possibility of entering the abdominal cavity and the possible risk of the laparoscopic operation itself; the actual recurrence rate is slightly higher because the mesh is not directly sutured and fixed; and the adoption of laparoscopy and general anesthesia increases the cost. In practice, laparoscopy is more suitable for patients with bilateral hernia, patients with recurrence after open hernia repair, patients who require a quicker return to work and patients who request laparoscopic surgery themselves. 2. On the contrary, open hernia repair is more obvious in terms of recent incision pain and relatively slow recovery. The incision is relatively long but only 4-5 cm, and the incision is in the groin which can be covered by pants; various anesthesia methods may be chosen according to the patient’s specific situation; the cost is lower than that of laparoscopic surgery; the suture is closed under direct vision, and the recurrence rate is lower than that of laparoscopic surgery in China. All things considered, unilateral incipient hernia is a more suitable choice for open hernia repair surgery; in addition, patients who have had major pelvic surgery are advised to choose open surgery.
