Nine notes on the use of insulin pumps

  Insulin pump is a high-tech precision medical instrument for intensive insulin treatment of diabetes. It is composed of a microcomputer chip, an ultra-micro motor and a reservoir with regular insulin, the size of which is like a pager, with a micro motor and an insulin reservoir inside. The special needle of the external infusion tube is stuck under the skin of human body for a long time, and the insulin is slowly injected into the subcutaneous short-acting insulin or ultra-short-acting insulin analogues according to the set program. The insulin pump simulates the continuous release of insulin into the body at different rates from the pancreatic beta cells of a normal person. It is infused into the body in two main ways, namely basal infusion and pre-meal infusion. Basal infusion is a 24-hour continuous small dose release of insulin set by professionals according to blood glucose monitoring, through this continuous small dose infusion can make patients’ fasting blood glucose stable, especially can maintain the stability of fasting blood glucose at night and early morning, which can effectively control the dawn phenomenon; before meal, adjust the pump according to the diet, set the pre-meal amount, control the post-meal blood glucose and make the blood glucose close to the normal level throughout the day.  Precautions for using insulin pump: 1. Have sufficient mental preparation and economic preparation before using.  In addition to the cost of insulin, consumables (insulin reservoir, connection tube, special injection needle, special adhesive material) should be replaced frequently, which costs about 100-200 yuan per month and is not included in the scope of medical insurance reimbursement. If you feel uncomfortable after spending money to use it or don’t want to use it with the needle on your body, you can’t return the insulin pump, and you will have spent tens of thousands of dollars for nothing.  2, insulin pump insulin is short-acting insulin or ultra-short-acting insulin analogues, can not use medium- or long-acting fish protein zinc insulin or ultra-long-acting insulin analogues. Short-acting insulin is a hexamer that breaks down into monomers in the body to function, which takes a certain amount of time, while ultra-short-acting insulin analogs are dimeric and break down into monomers quickly, so it is best to use ultra-short-acting insulin analogs with insulin pumps, which function quickly in the body and are closer to the physiological state.  3.Insulin pump injects insulin or insulin analogues into subcutaneous tissues, the absorption speed and absorption curve of insulin is similar to subcutaneous insulin injection with syringe, after the additional insulin before meal lowers postprandial blood sugar to the ideal level, the remaining subcutaneous insulin effect is still strong and can still cause hypoglycemia, so additional meal is still very necessary.  4. Insulin in solution is hexamer, which breaks down into monomers slowly, and ultra-short-acting insulin analogues in solution are dimer, which break down into monomers quickly, so when setting the 24-hour basal insulin injection distribution dose and the additional insulin dose before meals, the time for injecting short-acting insulin is 0,5 to 1 hour earlier than the time for injecting ultra-short-acting insulin analogues.  5.The concentration of insulin used in insulin pump is 100 units/ml, which is the same as the concentration of human insulin refill, and the concentration of ordinary bottled insulin is 40 units/ml, which cannot be used in insulin pump.  6.Replace the needle and connecting tube regularly as required to prevent infection and blockage.  7.The additional insulin key can be pressed according to the meal time, and the additional insulin dose can be adjusted at any time according to the amount of food eaten.  8.If in a stressful state, the basal insulin injection amount can be adjusted at any time, and when the stressful state gradually improves, the basal insulin injection amount should be adjusted at any time to avoid hypoglycemia.  9. Always ask the endocrinologist or the professional doctor at the after-sales service point to check the use of insulin pump and adjust the insulin dose.
