Abstract: The patient presented with skin rash and erythema along with recurrent itching and other symptoms, which are clinical signs of atopic dermatitis. After examination, it was found to be caused by a family history of asthma and food allergy. After systematic treatment, the patient’s skin damage symptoms were significantly improved and the pruritus symptoms were relieved, and the treatment effect was more satisfactory and the prognosis was better.
Basic information】Female, 2 years old
Disease Type】Atopic dermatitis
Hospital】Xiangya Second Hospital of Central South University
Date of Consultation】June 2022
Treatment Plan】Oral medication (Cetirizine Hydrochloride Drops) + topical medication (Halometasone Cream, Denide Cream, Zinc Borate Oxide Ice Ointment)
Treatment period】After 1 week of outpatient treatment, regular review
Treatment effect】After treatment, the patient’s itching symptoms were relieved and the dry skin symptoms were reduced.
I. Initial consultation
When we first saw the patient, the patient’s skin was dry all over the body, especially on the hands and feet, with obvious rash and erythema, accompanied by scaling and hypertrophy, and partly with chapped symptoms. The family told us that the patient’s skin rash and erythema often appeared since birth, accompanied by itching, and the symptoms were relieved by self-medication, but recurred. The patient’s grandfather had asthma, and the itchy symptoms had affected his sleep.
II. Treatment history
The patient’s allergen test results showed that the patient was mildly sensitive to corn, highly sensitive to egg whites/yolk, highly sensitive to milk, moderately sensitive to rice, highly sensitive to tomatoes, and highly sensitive to wheat. After the diagnosis was confirmed, I informed the patient’s family that medication could be used to combat allergies and relieve the symptoms of itching, and the patient’s family agreed, so the patient was prescribed cetirizine hydrochloride drops to be taken orally, which has anti-allergic effects and can help relieve itching and other symptoms, thus improving sleep. After the symptoms are relieved, the treatment is maintained by applying Ned’s cream. In addition, apply zinc oxide borate ice cream to the cracked skin to help heal the wound.
Treatment effect
The patient’s itchy skin was relieved and the dryness of the skin was reduced after the drug treatment. On the 3rd day of treatment, the patient’s dry skin symptoms improved significantly, the accompanying scaling symptoms disappeared, and the itching symptoms were improved. On the 7th day of treatment, the patient’s pruritus disappeared and the dry skin symptoms were significantly reduced, so the treatment effect was more satisfactory. After stopping the medication, the patient was rechecked at the hospital and there was no recurrence of the disease.
IV. Notes
I was very happy to see that the patient’s symptoms were relieved after treatment, and his skin was smooth and delicate again, and he could play happily. However, patients as well as their families should also pay attention to the following points in their daily life.
1. Families should pay attention to skin care for patients on a daily basis. They should pay attention to cleaning the patient’s skin with warm water, while trying to avoid using irritating soap, and applying some moisturizing emollients after cleaning the skin.
2, it is recommended that family members give patients to wear some loose, breathable clothes to avoid repeated friction stimulation induced aggravation of the disease.
3. Since the patient has more allergic foods, the family should also pay attention to avoid adding eggs, milk, wheat and other foods to the patient’s diet.
V. Personal insight
It is good that the baby girl was found early, and the consultation was timely. After systematic treatment, the symptoms improved and the prognosis was good with daily care and avoidance of food. However, if the condition is left unattended, the condition may worsen and skin infections, asthma, urticaria and other diseases may occur, as well as skin breakouts and other symptoms, resulting in skin scarring.
