Talking about Chinese medicine treatment of interstitial lung fibrosis

  Interstitial lung fibrosis is referred to as pulmonary fibrosis.  It is a disease characterized by diffuse alveolar inflammation and disruption of alveolar structure eventually leading to interstitial fibrosis of the lung. How does TCM recognize pulmonary fibrosis?  In traditional Chinese medicine literature, there is no disease name corresponding to pulmonary fibrosis, but most doctors classify it as “lung impotence”, “lung paralysis”, “cough”, “asthma”, “shortage of breath” according to its clinical manifestations. There is a controversy among famous Chinese medicine scholars in China about the name of pulmonary fibrosis in Chinese medicine, but there are basically two academic views with consensus. The theory of pulmonary fibrosis is derived from the “Treatise on Palsy”, “Suwen? The theory is derived from the “Treatise on Paralysis”, “The Five Viscera Generation”, “Suwen? The four times of thorns are discussed from the treatise. From the etiology and pathogenesis, it is believed that pulmonary fibrosis is an evidence of deficiency of the lung and kidney, and deficiency of both qi and yin of the lung and kidney. Many scholars believe that its pathogenesis is due to congenital deficiency, weak endowment, deficiency of positive qi, external evil attacking the lung, and obstruction of qi and blood flow, resulting in qi stagnation and blood stasis and loss of circulation of the veins. Some scholars believe that deficiency of lung qi and phlegm obstruction of the lung are the basic pathogenesis, and the more deficient the lung qi is, the more severe the phlegm stasis obstructs the lung, thus causing the onset and development of fibrosis. In short, lung and kidney qi, deficiency of qi and yin, phlegm, heat, toxicity and stasis blocking the ligaments are the main pathological mechanisms.  Second, how to break pulmonary fibrosis?  1, clinical symptoms such as patients often have shortness of breath, cough, chest tightness and other manifestations.  2, chest film and CT manifestation of reticular, corrugated grinding glassy shadow.  3.Pulmonary function tests show a significant decrease in ventilation and diffusion function.  4.It can be confirmed by lung biopsy.  What are the current treatments for pulmonary fibrosis? 1. Glucocorticoids, which are only effective in some types in about 20-30% of patients. 2. Immunosuppressive agents, such as cyclophosphamide and azathioprine, which are effective in some patients but have limited efficacy. 3.  3. antioxidants such as N-acetylcysteine, etc., which are effective when taken for a long time.  4. cytokine therapy such as leukotriene receptor antagonists, tumor necrosis factor, transforming growth factor, monoclonal antibodies, etc. are under study and their efficacy is not yet certain.  5, other treatment interferon, macrolide antibiotics research that can regulate immune cell activity, inhibit fibroblast proliferation and collagen synthesis, etc.. However, there is a lack of precise clinical basis for treatment of large samples.  6, Chinese medicine treatment.  Fourth, how to treat pulmonary fibrosis in Chinese medicine?  I. The basic treatment is to activate blood circulation and eliminate blood stasis, benefit qi and nourish yin, clear heat and moisten dryness to stop coughing, resolve phlegm and clear ligaments, tonify kidney and benefit lung, and soften and disperse nodules.  Commonly used therapeutic drugs are 1.Single Chinese medicine preparations such as Chuanxiongzin injection, Ginkgo biloba extract injection, Danshen injection, Leigongteng multi-glucoside tablets and other Ginkgo biloba capsules, etc.  2, compound Chinese medicine, such as: the preparation of turtle nail pill, anti-fibrous granules, blood province and blood stasis capsule, ginkgo biloba capsule, etc., 3, Chinese medicine soup evidence-based treatment according to the patient’s symptoms and performance dialectical typing treatment.    Huang Yao Zi and other drugs, add or subtract according to the type of evidence of the patient.
