Mr. Lu was operated in a local hospital at the end of April last year for a brain tumor, and the postoperative pathology diagnosis was astrocytoma grade II. 6 months later, the tumor was found to have recurred, and he underwent craniotomy again in a local hospital, and the bone flap was also removed, and the postoperative pathology diagnosis was still astrocytoma grade II. In order to prevent the tumor from recurring, his family brought him to our TCM hospital and asked for traditional Chinese medicine treatment for this brain tumor. It has been more than half a year since his treatment, and he has not seen any sign of tumor recurrence after repeated CT and MRI examinations. However, recently, Mr. Lu gradually developed mental impairment, depression and irritability, unstable walking, gradually unable to take care of himself, even bedridden, and the most distressing thing for his family was that he did not even know how to urinate or defecate. The family brought him to the original surgery hospital for several times for follow-up, and a review of the cranial CT revealed an obvious brain penetration deformity in the original surgery area, and the water in the ventricles reached under the skin and raised the scalp, just like a large bag on a goose’s head. Doctors at the original surgery hospital repeatedly punctured the scalp with a syringe to extract the water, and the bag on the head shrank at that time, but within a few days it grew again and got bigger and bigger. To make matters worse, the CT showed that the water in his ventricles was also accumulating more and more, and the ventricles were gradually getting bigger and bigger, compressing the surrounding brain. Mr. Lu’s symptoms were getting worse and worse, and his family was really anxious. After carefully studying his condition, we considered that Mr. Lu’s current symptoms were mainly related to hydrocephalus. Two surgeries at the outside hospital had opened up the frontal cortex to the ventricles, forming a penetrating malformation, and cerebrospinal fluid was leaking out of the ventricles, soaking the frontal lobe area and accumulating under the skin. The frontal brain tissue normally controls the person’s emotions and if damaged can cause mental retardation, memory impairment, and temperament changes. Gait instability and incontinence are also closely associated with intracerebroventricular hydrocephalus. In other words, the patient’s new set of symptoms are related to excessive accumulation of cerebrospinal fluid in the brain. In terms of treatment, simple puncture scalp drainage does not solve the underlying problem and can easily lead to intracranial infection. Comprehensive consideration suggested performing cerebrospinal fluid shunt surgery, which drains cerebrospinal fluid through a subscalp puncture placement in the original surgical area and then passes through a subcutaneous tunnel to the patient’s abdominal cavity, thereby draining excess fluid from the brain through the tube into the abdominal cavity, where the peritoneum has the ability to absorb fluid and can absorb the cerebrospinal fluid. After patient explanation of the condition, the patient’s family accepted this surgical option. The operation went well, and after the operation, the mass on Mr. Lu’s forehead was gone, and he gradually became self-aware of his bowels, and slowly he was able to walk on the ground again and basically take care of himself. Mr. Lu started to smile, and his family members who had been suffering from heavy nursing work around him also showed their long-awaited smile. As we all know, glioma is the most common type of brain tumor, accounting for 40.49%, and the most common type of glioma is astrocytoma. The treatment is mainly surgery + radiotherapy + chemotherapy, but the overall efficacy is poor and the tumor recurs almost 100%. After the opening of brain tumor clinic in our hospital, using the combination of Chinese and Western medicine, many patients’ tumors have been controlled, their quality of life has been improved, and their life expectancy has been extended. Traumatic brain injury, cerebral hemorrhage, brain tumor, and inflammation of the brain can lead to excessive secretion of cerebrospinal fluid or (and) impaired circulation and absorption, resulting in increased intracranial cerebrospinal fluid volume, enlargement of the ventricular system or (and) enlargement of the subarachnoid space, which results in hydrocephalus. Patients may experience symptoms such as gait disorders, mental disorders, and urinary and fecal incontinence. After the opening of our specialized hydrocephalus clinic, the use of Chinese and Western medicine combined with surgical treatment can significantly relieve symptoms and reduce or even eliminate hydrocephalus. The special feature of this patient is that he has both brain tumor and hydrocephalus. After the series of Chinese and Western medicine treatment, the tumor was controlled and the hydrocephalus was relieved, which improved the quality of life of the patient. The patient and his family were very satisfied. Our medical staff also felt very pleased.
