Beware of neurological tinnitus

This year’s Qingming Festival, the old Li returned to the countryside to visit the tomb, in the process of sweeping the tomb, always hear the sound of firecrackers one after another, suddenly felt that the right ear heard a machine rumbling sound, immediately like a stuffed cotton in the ear, hearing is also greatly reduced. Later, slowly unable to hear the words of the family, even close at hand, it is difficult to hear, and the volume of speech unconsciously increased, into a loud public. After returning to Qingyuan, Lao Li hurriedly found the hospital ENT department. After examination, Lao Li suffered from neurological tinnitus. 1. Symptoms: Abnormal sound sensation without external stimulation The so-called neurological tinnitus refers to the abnormal sound sensation produced by people without the influence of any external factors. For example, people may feel the sound of cicadas, buzzing and hissing in their ears, but in fact there is no corresponding sound in the surrounding environment, which means that tinnitus is only a subjective feeling. Tinnitus can be transient or persistent. Severe tinnitus can be very disturbing and stressful. If the tinnitus is transient and comes and goes, it is generally a physiological phenomenon, so you do not need to be overly nervous and can just let it go. If the tinnitus is persistent, especially if it is accompanied by other symptoms such as deafness, vertigo and headache, you should be alert and seek medical attention as soon as possible. The symptom of neurogenic tinnitus is tinnitus vertigo, which is often the most obvious sign of deafness. The next manifestation is gradual hearing loss, and most patients will only complain that they cannot hear well. By inaudible, it is actually a loss of hearing. Then there is interruption or asking the other person to repeat themselves, and when talking face-to-face, they often ask the other person to raise the volume. And the volume of their own speech will also be unconsciously increased. Another manifestation is a lack of mental concentration and changes in auditory attention. For example, it is not easy to focus on listening to the other person’s conversation after a long time. Often do not notice that others are greeting them. 2. Etiology: storm sound and prolonged noise exposure What are the factors affecting tinnitus? Diseases of the ear, such as external ear diseases: otitis externa, foreign bodies in the external ear, etc., can cause tinnitus. Nasal diseases, such as deviated nasal septum and enlarged inferior turbinate, can also cause tinnitus. Vascular diseases can also cause tinnitus, such as dilation of small blood vessels in the ear, vascular malformation, hemangioma, etc. The tinnitus from veins is mostly noisy. Some other systemic diseases can also cause tinnitus. For example, lack of blood supply to the brain, pre-stroke, hypertension, hypotension, anemia, and diabetes. In addition, tinnitus and hearing loss can also occur from overuse of drugs that have toxic effects on the ear such as gentamicin, streptomycin or kanamycin. Excessive fatigue, lack of sleep, and excessive emotional stress can also lead to tinnitus; stormy sound and prolonged noise exposure can lead to hearing loss and tinnitus; caffeine and alcohol can often aggravate tinnitus symptoms; smoking can make blood oxygen drop, and inner ear hair cells are an extremely sensitive cell to oxygen, so lack of oxygen can cause damage to hair cells, so attention should be paid to changing bad habits. 3. Prevention: prevent dirt from entering the ear to keep the outer ear canal clean and hygienic How to prevent neurogenic tinnitus? First of all: plugging the ears and bathing, preventing dirt from entering the ears, and keeping the outer ear canal clean and hygienic is an important part of preventing ear disease. Dive less in swimming pools to prevent sewage from entering the nasopharynx to prevent bacteria from entering the middle ear. If you dig your ears with hard objects such as iron, wood or bone, it is easy to cause trauma to the ear canal or even the eardrum, which can cause pain, bleeding, swelling or hearing loss. In addition, children should be taught not to put beans, food, confetti or insects into the ear canal to avoid foreign bodies in the ear canal. If there is pain in the ear canal, hearing impairment, or even pus flowing from the ear canal, prompt examination and removal of the foreign body is required. There is also the following: entertainment is moderate and the volume is appropriate. The eardrum is very thin, and if the volume is too high, it is vulnerable to damage. Therefore, MP3, television, tape recorders, audio volume should be moderate, the tone should not be too high or too low. If you work in a noisy environment for a long time, you should wear noise-proof earplugs and have your hearing checked regularly. 4.Treatment: Commonly used vasodilators to improve inner ear blood circulation Current treatment methods include medication: commonly used vasodilators to improve inner ear blood circulation, such as ginkgo biloba; or drugs to improve energy metabolism of inner ear tissues; anticonvulsants, such as carbamazepine, cipro, lidocaine, etc. can inhibit the abnormal excitatory activity of the auditory system. Masking therapy: Tinnitus masking device is used to treat tinnitus, which is the use of external acoustic stimulation to inhibit the spontaneous excitation of the inner ear or the auditory nerve; hearing aids can also be used instead of masking devices. Psychotherapy: Keeping a happy mood and diverting attention may reduce tinnitus.
