Meniscal injuries of the knee cannot be easily removed

  Meniscal injuries are a very common type of sports injury in sports, accounting for approximately 50% of knee injuries. It is caused by the uncoordinated movement of the joints during sports, resulting in the meniscus getting stuck between the femur and tibia under abnormal stress and tearing. Patients with meniscus injuries feel weakness and pain in their lower limbs when walking, especially when going up or down stairs, and often hit a weak leg, which affects their work and life.  How to treat after meniscus injury?  Before we talk about specific treatment methods, it is necessary to understand the biomechanics, histological characteristics and functions of the meniscus. In knee flexion and extension activities, the anterior and posterior angles of the medial and lateral meniscus are moving anteriorly and posteriorly with the tibial plateau, while in knee rotation movements, the body of the medial and lateral meniscus is moving with the femoral condyle. Therefore, if we add torsional movements to the flexion and extension movements of the knee, our meniscus is susceptible to injury. The meniscus is histologically classified as a fibrocartilage tissue, which is less able to heal itself after an injury, making it more difficult to treat.  However, the meniscus plays an important role in the function of our knee joint: 1) it absorbs shock, acts as a shock absorber and cushion, and protects the articular cartilage; 2) it acts as a ball to improve the morphological match between the femur and tibia, making the knee joint more tight and stable; 3) it conducts joint stress and prevents excessive anterior slippage of the femur; 4) it prevents excessive extension and rotation of the knee joint; 5) it can secrete synovial fluid to lubricate the joint.  Once the meniscus is damaged, these functions can be lost, even causing irreparable damage to the knee joint. In the past, due to the limitation of treatment technology, doctors were unable to repair the damaged meniscus and had to remove it. However, after the meniscus is removed, the lack of protection of the meniscus soon leads to untreatable osteoarthritis, which causes persistent pain in the knee and even impairment of joint movement.  Especially in adolescents, if the meniscus is removed because of a simple meniscal injury, it is more likely to cause re-injury to the knee joint, making it difficult to maintain a normal life and work, and very difficult for doctors to treat. Therefore, it is important to treat meniscus injuries in a timely manner, and the treatment must be preserved as much as possible and not easily removed!  Most meniscus tears of the knee must be treated surgically. The surgical treatment of the meniscus has also undergone a process of constant updating: from the earliest total removal to later partial removal to the current suture repair. Arthroscopic suture repair of the meniscus is a surgical procedure that preserves the damaged meniscus to the greatest extent possible, but it heals differently after repair because of the different blood supplies to different areas of the meniscus.  The meniscus can be divided into red-red zone (rich blood supply zone), red-white zone (not rich blood supply zone), and white-white zone (lack of blood supply zone) according to the blood supply. Therefore, when we repair the torn meniscus with sutures, we also have to take into account the blood supply of the injured area so that we can adopt some methods to promote postoperative healing.
