Strategies of anti-tumor metastasis treatment with Chinese medicine

Combined with the modern research on tumor metastasis, we discuss the treatment ideas and methods of Chinese medicine against tumor metastasis: we believe that the treatment of Chinese medicine against tumor metastasis should focus on improving the immune function of the body, improving the hypercoagulable state of blood and microcirculatory disorders, tethering thrombus formation, reducing tissue edema, and enhancing the effect of drugs to directly inhibit and kill cancer cells, and at the same time, we attribute the pathological mechanism of tumor metastasis to deficiency, stasis, toxicity, phlegm, dampness, etc., and put forward The main methods of anti-tumor metastasis treatment in TCM were proposed as the methods of supporting and cultivating the root, invigorating blood to resolve scabies, clearing heat and detoxifying toxins, resolving dampness and diuresis, softening hardness and dispersing nodules, and attacking toxins with toxins, etc. The current problems in anti-tumor metastasis research were also analyzed. Metastasis is a major obstacle to cancer treatment, and controlling metastasis is a key factor in determining the prognosis of cancer patients [1]. In recent years, foreign countries have made great progress in tumor metastasis research, elucidated certain specific aspects and mechanisms of metastasis, and developed metastasis-preventing drugs, such as propionamide, carbamide monoamine monoimidazole, etc., but there is not yet a drug widely used in clinical metastasis prevention. Therefore, it is important to actively explore the ideas and methods of tumor metastasis prevention treatment in Chinese medicine and make efforts to develop metastasis-preventing Chinese medicines. In this paper, combining the progress of modern medical research on tumor metastasis and certain achievements of Chinese medicine in the treatment of tumor, we only talk about our personal understanding and vision on the therapeutic ideas, treatment methods and problems in the research of Chinese medicine against tumor metastasis. 1. The elucidation of the process of tumor metastasis, related factors and treatment strategies proposed by modern medicine provide research ideas for the anti-tumor metastasis treatment of TCM Modern medicine believes that tumor metastasis is a very complex, multi-step continuous process, including cancer cells shedding from the primary tumor, invading adjacent tissues, entering the circulatory system, penetrating the basement membrane, infiltrating the surrounding tissues, and growing in the secondary sites to form metastases. Among them, motility factors and their receptors, matrix degrading enzymes, information transmission of metastasis, oncogene defects, metastasis-related genes and metastasis suppressor genes (NM23, etc.) are involved in the metastasis process, and various therapeutic approaches have been developed for different aspects of metastasis, including anti-migration mechanisms, inhibition of extracellular matrix degradation, anti-adhesion, blocking information transmission, and inhibition of angiogenesis, etc. have been effective. The mechanism of tumor metastasis is certainly complex, but it can be considered that the formation of metastatic clones is the complex result of a series of interactions between tumor and host, including both tumor factors, such as the biological characteristics of malignant tumor cells, and host factors, such as the conditions of the microenvironment of organ tissues for the growth of tumor cells and the regulatory effects produced by organ tissues. Tumorigenesis to metastasis formation is not easy, and some experiments have shown that 24H of tumor cells per gram of tumor tissue can release (3.2X1.4)×lo6) into the blood, but ultimately less than 1% survive and form ? less than 1% of metastatic tumors [2]. Once the tumor cells enter the vasculature, they must escape the attack of the host immune cells and the physical impact of blood pressure and blood spin, etc., and the vast majority of cancer cells die. Analyzing the conditions in the blood and lymphatic circulation that are unfavorable for the survival of tumor cells (microcirculation V structure, function, coagulation, anticoagulation factors, etc.), searching for anti-metastatic factors in the host blood circulation that can be reliably resorted to, such as immunologically active cells (NK cells, macrophages, killer T cells, etc.), and exploring and applying the patient’s own immune system that exists, have good and wide application in today’s anti-tumor metastasis treatment strategies It also provides ideas and clues for the anti-tumor metastasis treatment of Chinese medicine. 2. Awareness of the etiology and pathogenesis of tumor metastasis in Chinese medicine The chapter of “The Beginning of All Diseases” says: “Staying and not going, passing outside the gastrointestinal area, between the original collection, staying in the veins, staying and not going, resting and becoming accumulation.” The one who stays is also a tumor. This is (probably) the earliest record of tumor metastasis in Chinese medicine. The understanding of the pathogenesis of metastasis by successive generations of physicians mainly includes qi stagnation, blood stasis, phlegm coagulation, toxin gathering and internal deficiency of positive qi. Modern TCM scholars combined with the research results of Western medicine, have a new understanding of the etiology and pathogenesis of tumor metastasis recurrence. Xu Zhenclow et al. from the Department of Oncology, Longhua Hospital, Shanghai, believe that low immune function is the key to the recurrence and metastasis of malignant tumors, and that Chinese medicine can improve cancer immune function and reduce the surface active substances of tumor cells [3]. Qiu Jiaxin et al. believed that gastrointestinal tumors are particularly frequent in the state of spleen deficiency, and that strengthening the spleen and benefiting the qi can control the metastatic recurrence of tumors [4]. According to Yu Rincun of Beijing Hospital of Traditional Chinese Medicine, the recurrence of tumor metastasis is related to the loss of balance in the internal environment, such as the decrease in the function of human internal organs, the imbalance of qi, blood, yin and yang, the deficiency of positive qi, and the decrease in the body’s ability to resist disease, and that internal obstruction of blood stasis (similar to the hypercoagulable state of blood in modern medicine) is an important factor in cancer recurrence and metastasis, and that blood-stasis activating drugs are important to prevent recurrence and metastasis [5]. Liu Zilong et al. believed that among the factors of recurrence and metastasis, the deficiency of positive energy and the failure of positive to suppress evil are the decisive factors, together with the mutual gelation of phlegm, stasis and toxicity, and the liver depression and spleen deficiency promote the occurrence of metastasis. Therefore, the organic combination of the methods of supporting positive energy, eliminating evil and attacking toxins, dissolving phlegm and dispersing stasis, and relieving liver and depression will control cancer recurrence and metastasis more effectively [6]. 3. Anti-tumor metastasis treatment methods of Chinese medicine 3.1 Fu Zheng Pei Ben method: Cancer patients have low immune function, which makes it difficult for immune active cells to recognize and kill cancer cells with metastatic ability in blood microcirculation, leading to the formation of metastasis. Fu Zheng Pei Ben medicine can regulate the immune function of the body and improve the activity of various anti-cancer cells and factors of the body, mainly including improving the phagocytosis function of macrophages, activating the activity of lymphocytes, improving the activity of NK cells and LAK cells, etc., so as to enhance the ability of immune cells and factors to attack cancer cells in blood and kill as many cancer cells as possible. In addition, the enhancement of local immune function of tissues can reduce the number of cancer cells that detach from the primary foci and enter the circulation, thus controlling or slowing down the formation and development of metastases. Fu Zheng Pei Ben drugs also have the effects of improving the hematopoietic function of bone marrow, enhancing the regulatory function of internal fraction fluid, and regulating the intracellular cAMP content and cAMP/cGMP ratio, thus affecting the conditions necessary for the growth of tumor cells. Therefore, the method of supporting and cultivating the root is the most important method of anti-metastatic treatment in Chinese medicine. This method can be applied throughout the whole process of tumor prevention and treatment. The specific methods include benefiting Qi and nourishing Blood, nourishing Yin and generating fluid, nourishing Yin and filling essence, warming Yang and nourishing kidney, strengthening spleen and stomach, softening liver and nourishing liver, etc. The focus is on strengthening spleen and benefiting kidney. 3.2 Blood activation and elimination of blood stasis method: Studies on tumor blood obsession evidence have confirmed that tumor patients have pathological changes such as increased blood viscosity, abnormal blood rheology and microcirculatory disorders. The existence of blood hypercoagulability makes micro thrombi including cancer cells easily formed in micro vessels, thus cancer cells are killed by chemical drugs and immunologically active cells. The adhesion of tumor cells to capillary endothelium during metastasis and the formation of neovascularization in metastases are also related to the hypercoagulable state of blood. Platelets play an important role in thrombosis and their aggregation has a facilitating effect on the occurrence of cancer metastasis. Activating drugs can directly inhibit and kill tumor cells, change blood rheology, reduce blood viscosity, anticoagulate, inhibit blood… Platelet activity, pro-fibrinolysis, anti-thrombotic, and elimination of microcirculatory disorders. It can put cancer cells under the monitoring of anti-cancer drugs and immune function of the body. Therefore, the method of activating blood circulation and removing blood stasis is also one of the important methods of anti-metastasis treatment. 3.3 Clearing heat and detoxification method: malignant tumors, especially those with metastasis in the middle and late stages, often have fever, enlarged masses, local burning, pain, thirst, constipation, red tongue with yellow coating and counted pulse, which are all symptoms of evil heat and stasis. Modern research has confirmed that some heat-clearing and detoxifying drugs have anti-cancer effects, such as half a branch of lotus, white flowered snake tongue herb, dongling grass, etc. can directly or indirectly inhibit and kill cancer cells. Generally speaking, heat-clearing and detoxifying drugs have antibacterial, anti-inflammatory, antipyretic and symptom-improving effects. Therefore, heat-clearing and detoxification method is also an important method of anti-metastasis treatment. 3.4 Dampness and water-relieving method: Basic research found that tissue edema is a common phenomenon of tumor invasion and metastasis. On the one hand, edema of tumor tissues decreases the polymerization force among tumor cells, which is favorable for cancer cells to enter the operation process out of the mother body; on the other hand, edema of healthy tissues can separate the fibrous components, widen the tissue gap and weaken the resistance of tissue structure, which is favorable for metastatic cancer cells to invade and occupy. The application of spleen-reinforcing and dampness-reducing drugs can reduce tissue edema, which may provide an effective way for anti-metastatic treatment. Therefore, it is worthwhile to conduct in-depth research on the method of resolving dampness and diuresis in anti-metastasis. 3.5 Softening and dispersing nodules and attacking toxins with toxins method: its mechanism of action is mainly to directly inhibit and kill cancer cells, and the application of these Chinese medicines may directly kill tumor cells in the process of metastasis. Some traditional Chinese medicines can also activate macrophages and promote their phagocytic function to reduce the chance of metastasis. Tumor metastasis is a complex disease mechanism with multi-system, multi-tissue and multi-organ involvement, mixed cold and heat, and mixed deficiency and reality, so anti-metastasis treatment needs to be applied in combination with the above-mentioned methods and based on the method of supporting and cultivating the root. 4. Problems faced by the research of anti-metastatic treatment in TCM Two basic conditions should be possessed by clinically effective and experimentally proven anti-metastatic drugs [7]. At present, clinical research on anti-metastasis in TCM has greater difficulties. One, clinical designs generally require large samples and long observation times. Second, the factors associated with metastasis are complex, and even for tumors with the same stage and pathological type, their metastases are related to the type of surgery, the type of postoperative combination therapy, time, dose, duration of treatment, and their own condition, so it is also quite difficult to achieve prospective paired observation. Third, the monitoring of metastases is the primary indicator of clinical research on anti-metastasis. The time, site, and pathological type of metastases, especially the appearance of micro-metastases, require advanced instrumentation for timely detection, and the acceptance of patients also brings difficulties to the specific operation. Because of this, clinical research on anti-metastasis started late, and there are even fewer clinical reports on this aspect in TCM. The author’s research on anti-tumor postoperative metastasis, organized by the method of benefiting Qi and blood circulation and detoxifying and dispersing nodules, has achieved a stage of progress [8], but there are still many problems that must be further analyzed and discussed in depth. In recent years, more experimental studies on anti-metastasis in Chinese medicine have been conducted and certain achievements have been made, but the experimental study on anti-metastasis is only a part of the study of metastasis, which can explain limited problems and cannot yet imitate the whole process of metastasis, and many common clinical metastatic sites have not yet been established as animal models. In addition, the results of experimental studies do not necessarily correspond to clinical findings. For example, some animal experiments have confirmed that certain blood-activating and stasis-transforming drugs (such as salvia) can promote the metastasis and spread of cancer cells, but this view has not been confirmed by clinical data; on the contrary, many clinical reports have confirmed their inhibitory or delaying effects on metastasis formation. Therefore, experimental research and clinical research should be closely integrated and targeted rather than rigidly applied. At present, modern medical research on tumor etiology and pathogenesis is deepening from molecular biology, oncogenes and oncogenes. The application of modern scientific research methods and approaches to study the effects of TCM on malignant expression of cancer cells, enzymatic changes of target cells, the effects of oncogenes and oncogenes, the effects of DNA and RNA in the nucleus and the regulatory effects of many immune factors will be helpful to elucidate the effects of TCM on cancer cells. This will greatly contribute to the elucidation of the pharmacological effects and efficacy mechanism of TCM in anti-tumor and anti-metastasis.
