How can diabetes be prevented and treated?

With social development and lifestyle changes, diabetes has become a common and frequent disease in modern times. In China, the high incidence rate, high complication rate, high disability rate and low awareness rate, low treatment rate and low compliance rate of diabetes are relatively serious. It has a great impact on people’s health and social economy, and has become a major public social problem. Today is the World Diabetes Day, the author engaged in clinical work for 40 years, in the prevention and treatment of diabetes work slightly experience to discuss the following: A. The health risks of diabetes Diabetes is an ancient disease. The ancestral medicine in 3000 years ago discussed the thirst and similar to it. It has been clarified in modern times that its onset is closely related to genetic and environmental factors. It is a systemic chronic disease caused by relative or absolute insulin deficiency in patients, resulting in disorders of sugar, fat, protein, water, salt, acid-base metabolism. It makes the body not only raise blood sugar, but leads to the development of various complications. Among the acute complications ketoacidosis, hyperosmolar syndrome has a high mortality rate, which was 100% before the discovery of insulin. Even in the United States, which is currently medically advanced, the mortality rate is 10-30%, and up to 50% or more in people over 65 years old. The situation is even more serious in our country because of the lack of knowledge about diabetes, undiagnosed and untreated patients with their first emergency, dangerous disease and easy misdiagnosis. Among the various chronic complications of diabetes, the damage to various target organs is more and more prevalent, and the consequences cannot be ignored. For example, the incidence and death rate of diabetes combined with coronary heart disease, angina pectoris, arrhythmia, and myocardial infarction is 3-6 times higher than that of non-diabetic patients. Therefore, it is recognized that diabetes mellitus is a critical condition for coronary heart disease. The percentage of diabetic patients with complications of cerebral atherosclerosis, cerebral atrophy, cerebral infarction, cerebral thrombosis, and cerebral hemorrhage is about 60%. The resultant aphasia, dementia, paralysis and mortality are also very serious. Eye lesions are one of the most common complications of diabetes. It is well documented that diabetes is 10-25 times more likely to cause blindness than the general population. And diabetes causes nephropathy, which causes uremia 17 times higher than the general population and has become the leading cause of dialysis and kidney transplantation today. The vascular neuropathy of diabetes makes small wounds cause big damage, triggering gangrene and amputation 20 times more likely than the general population. In addition, diabetes causes impotence, constipation, sleep disorders, depression, skin infections, and urinary and genital tract infections. For patients, it is not only unbearable pain and unbearable itch, but also a lingering worry. The combination of diabetes mellitus with hypertension, hyperlipidemia, hyperuricemia (gout), fatty liver, and obesity is more common [3]. Patients must face daily visits to the doctor for medication and spending money on consumption, which seriously affects their lives and greatly diminishes the quality of survival and family happiness. Therefore, it is said that once you have diabetes, you are uncomfortable from head to toe; uncomfortable from physical to psychological; abnormal from eating and sleeping to pooping and urinating, in endless troubles. If you believe in false advertising and take counterfeit drugs, not only will you miss treatment, but you will also be left with no money. Second, the socio-economic impact of diabetes diabetes damage the physical and mental health of patients, and the impact on their families is also very obvious. Patients take medication for a long time and the medical cost increases a lot. It affects the family economy and relationship, and the instability factor rises. Moreover, since the beginning of the new century, the trend of younger onset of diabetes is becoming more and more obvious, and the number of children and adolescents with type 2 diabetes has increased greatly. Many of the most strenuous pillars of the family or the hardest and most troublesome unit leaders who contribute the most are more likely to develop diabetes. They are busy with their work and fail to treat it in time under pressure. Once the disease develops, it is very serious, and even die young. The result is a major loss to the individual to the family to the unit to the cause, and a great impact on social harmony and economic development. Many patients are already suffering from complications by the time they are diagnosed with diabetes, and the huge medical costs are a considerable burden. According to the city’s preliminary estimates, the cost of outpatient treatment for ordinary diabetic patients is about 2,000 yuan a year, and 5,000 yuan if combined with hypertension, etc.; if complications remain frequent, multiple hospitalizations or strokes, blindness, dialysis, etc., will inevitably increase the cost of treatment and care, which will cost at least 30,000 yuan to more than 100,000 yuan a year. Early prevention and treatment and serious illness and then treatment of the results are completely different, the amount of medical costs are also a world of difference. It is not uncommon for diabetes to cause disability and poverty due to the disease, dragging down the whole family. Since the beginning of the new century, the incidence of diabetes in China has increased rapidly, and the number of people with the disease has long been the second largest in the world. It is currently estimated that there are about 90 million people, and the prevalence rate in some major cities has reached about 10%. There is also about double the “reserve army” of diabetics with impaired glucose tolerance, who may join the ranks of diabetics at any time [4]. With a total population of 300,000 in the city and a prevalence rate estimated at a low level of 6%, the number of diabetics may be close to 20,000. The direct medical costs and indirect medical costs are staggering figures. Thus, it is said that diabetes is not a simple disease and has become a serious public health problem in today’s society. It must be taken seriously and treated seriously. On December 20, 2006, the 83rd United Nations General Assembly adopted the proposal submitted by the Group of 77 and China, which was confirmed by the United Nations Resolution 225. Considering that diabetes is a chronic, debilitating and costly disease with severe comorbidities that poses a serious threat to families, Member States and the world, and represents a serious challenge to the achievement of the development goals of the century. Decides to designate 14 November each year as World Diabetes Day at the United Nations, beginning in 2007, and requests all Member States, relevant agencies of the United Nations system and other organizations to carry out activities in an appropriate format. Raise public awareness of the prevention and treatment of diabetes and its complications through education and mass media. There is an urgent need for multifaceted efforts to promote and improve the health of the population and to provide methods of treatment and health education. Member States are encouraged to develop national policies for the prevention and treatment of diabetes that are consistent with the development of their health care systems and integrated into national development goals [5]. The 17th Party Congress established a people-centered approach and put forward the goal of accelerating the development of social undertakings, improving people’s lives comprehensively, and providing health care for all. According to the current situation of diabetes epidemic in our city and personal clinical experience, several ideas are proposed to discuss; 1. Raise awareness and pay attention to the prevention and treatment of diabetes. A family a unit or an area, to grasp the economy grasp income is correct. It is also important to understand the relationship between health and economy, which has a huge impact on social development. Some patients are never examined and treated, avoiding treatment until serious complications arise, then they panic and blindly use drugs, believing that there is something to break the root of the fake medicine, and regret after wasting money. Although diabetes is a systemic lifelong disease, it is not possible to cure it at this time. However, early prevention, early detection and early treatment can prevent or delay the onset of complications in many high-risk groups. It is possible to work as usual and live a long and healthy life with the disease without spending money or with less money. Its economic and social benefits are very obvious. To put an end to illegal drug advertising, scientific and rational use of drugs. Implementation of prevention-oriented, Chinese and Western medicine combined, diet therapy, exercise therapy and drug combination, the idea of Chinese medicine to treat the disease has a very important significance in the prevention and treatment of diabetes. 2, strengthen the leadership, the establishment of diabetes prevention and treatment institutions. Prevention and treatment of diseases that have a significant impact on the social population, the direct effect is to significantly reduce the prevalence of the disease, reduce the occurrence of complications and avoid the pressure of huge medical costs on the social economy. The indirect effect can improve the health of the whole population and promote the harmonious socio-economic development, and its great social significance and huge economic benefits are self-evident. Therefore it is necessary to set up municipal leading groups and research institutions for diabetes prevention and treatment, including collaboration among finance, health, science and technology, labor security, insurance, medical insurance, and agricultural cooperative medical departments, to jointly attack the prevention and treatment of diabetes, introduce policies, and act together. Screening of key high-risk groups, from basic care, to standard care, comprehensive care, so that the majority of diabetic patients are benefited. Countless facts prove; controlling blood sugar up to the standard, the risk of various complications can be greatly reduced, so that many patients benefit. Spending one dollar on prevention of disease plays the benefit of spending hundreds or thousands of dollars less on treatment of disease. Therefore, there is a need for census and prevention education for the diabetic population to strengthen life interventions and change unhealthy lifestyles with good effect. Radio and television, newspapers and books and related materials, and literary propaganda can be used to give the knowledge of diabetes prevention and treatment to the people. Everyone understands the causes of diabetes, everyone is clear about the prevention and treatment methods, you can play a half-hearted role. 3, scientific prevention and treatment, the establishment of a sound city, village and street community three-level medical prevention and treatment network. The limited medical resources can be effectively integrated and managed to maximize the effect. Provide individualized, long-term follow-up and multidisciplinary collaboration based on the three-level prevention and treatment model that gives full play to patient motivation. To reduce patients’ dependence on hospital doctors and unnecessary costs while ensuring the quality of care. The primary and secondary prevention work is focused on the outpatient community, while the tertiary treatment work is undertaken by the hospital inpatient department. Change the status quo of emphasizing treatment over prevention and education. Effective use of existing health resources, economical and reasonable use of the three-tier prevention network, the realization of the true meaning of two-way referral, reduce the economic burden of patients and their families and society as a whole, both to benefit patients, but also in line with the basic policy of national health security. In conclusion, the prevention and treatment of diabetes is related to the health of patients and the well-being of their families, and more importantly, it affects the socio-economic development. The current medical model still needs to be reformed and improved. the DCCT study proves that good glycemic control can save medical costs, stabilize family relationships, and promote harmonious social development. If we innovate the diabetes prevention and control system with a developmental perspective, we can also expand to the prevention and control of hypertension, hyperlipidemia, gout, cardiovascular and cerebrovascular diseases and tumors. So that no disease can be prevented early and minor diseases can be treated early, benefiting thousands of families. We hope that with active hard work, we can reduce the prevalence of diabetes. Scientific and rational treatment of diabetes, reduce the development of complications, and improve the health of the people in our city. People live and work in peace and happiness, social harmony and health, and rapid economic development can be achieved as soon as possible.
