What to do about perineal and sacrococcygeal pain

  The sacrococcygeal region is one of the most common sites of pain. It is more common in women. Sacrococcygeal pain is commonly seen after abdominal pelvic tumor surgery, after anal perineal trauma or surgery, caudal fracture, anal scarring, etc. The clinical symptoms of perineal pain are complex and can manifest as urethral pain, vaginal pain, perianal pain, etc. It is also known as anal perineal pain, etc. The nature of pain can be burning, pins and needles, twitching, tearing, etc. The pain varies in severity and can be triggered or aggravated by urination, defecation, sexual intercourse, etc. Patients are usually too shy to seek medical attention, which has a great impact on their body and mind. Conservative treatment alone is ineffective for this type of intractable pain that originates from the sympathetic nervous system. Patients suffer a lot.  The anatomy of the perineum is complex, with innervation from the pubic, ilioinguinal and genitofemoral nerves, and sympathetic fibers from the sacrococcygeal part of the sympathetic chain. The lumbar sympathetic nervous system consists of two sympathetic fibers with ganglia that converge in the odd ganglion anterior to the sacrococcygeal joint, an important sympathetic ganglion that innervates the pelvic floor organs. Variations in the manifestation and location of the odd ganglion, with differences in shape and size, partly in the sacrococcygeal joint and also in the coccyx, and variations in these factors, may be the cause of the poor block effect.  CT-guided odd ganglion block and radiofrequency modulation were used, and good results were achieved. As explained by Chinese medicine, there is a point under the end of the caudal bone where the Governor’s Vessel starts – Changqiang point. The Directing Vessel is the sea of Yang meridians. Damage to the source of the Directing Vessel is obstructed, disturbing the mutual constraints of the meridians in the body. “When yin and yang are secret, the spirit is cured”, and when yin and yang are out of balance, miscellaneous diseases arise. Therefore, the regulation of the Qi ganglion has good curative effect in the long run.
