  In many years of clinical work, we found that tumor patients are mostly accompanied by obvious emotional and mental disorders, which are mainly of two types: one is anxious type, extremely easy to get angry and anxious, extroverted performance; the other is depressed type, superficially calm, internally thinking a lot, not enough communication with others, introverted performance. Both types can significantly promote the occurrence and development of tumor to different degrees and are closely related to the prognosis of patients. Therefore, the regulation of emotion is of great importance for patients’ recovery, among which the role of Chinese medicine in this area should be said to be very advantageous. Here is an article written by our group combined with our recent research for your reference!  In recent years, the establishment of the biopsychosocial medical model has prompted scholars to pay attention to the role of social, environmental and psychological factors in the disease when exploring the etiology of tumors. Among them, psychological factors, i.e. emotional factors, play an important role in the development of tumors. This article elaborates from the theoretical basis to the clinical application of the method of dredging liver and Qi in the treatment of tumors, in order to provide reference for clinical workers.  The seven emotions are based on the function of internal organs. Too much or not enough of them will damage the function of internal organs, and prominently cause the disturbance of qi, thus participating in the occurrence, development and transformation of tumor. In a large number of ancient texts, medical practitioners have recorded that internal injuries caused by emotion and will cause tumor due to obstruction of qi and disturbance of qi and blood, for example, “Nei Jing” says: “If you are suddenly hit by cold outside, if you are injured by worry and anger inside, the qi will be reversed …… and the blood will not be dispersed, and the fluid will be concubitated. Clinical practice proves that malignant tumor is a strong psychological stress for patients, and has its unique psychological characteristics, such as cognitive and emotional manifestations of depression, lack of interest, irritation, low self-evaluation or guilt, difficulty in concentration, suicidal thoughts and other clinical manifestations of depression, modern medicine has studied the connection between these clinical symptoms and tumor from the molecular biology level, and now it is believed that the mechanism of action is related to the neuro-endocrine-endocrine mechanism. The mechanism of action is thought to be related to the alteration of neuro-endocrine-immune regulatory network, psychological stress and pain stress, melatonin mechanism, retinoid nuclear receptor mechanism, cytokine mechanism and DNA damage and repair mechanism.  2. Theoretical basis of liver and qi treatment 2.1 The liver is most closely related to the seven emotions that cause diseases. For example, Su Wen? The theory of raising pain” says: “I know that all diseases are born from qi. Anger is gas on, happy is gas slow, sad is gas …… anger is gas reversal, even vomiting blood and food discharge”. And no matter what kind of emotional disorders, can eventually lead to disorders of the qi. The liver likes to be organized and is not depressed, the main drainage, the main blood collection. Liver Qi is the basis for the smooth flow of Qi in the five viscera and six bowels, the seven emotions are more anger, and anger is more likely to hurt the liver. If the emotions are not smooth, the anger is not stretched, and the desire is not fulfilled, resulting in liver qi stagnation, qi disorder, unfavorable lift and discharge, and abnormal drainage, not only the internal organs become diseased, but also affect other internal organs, causing a variety of symptoms, which is quite common in clinical practice; as Wang Mengying said, “the disease of the seven emotions must start from the liver”. It is said in the “Reading Medical Essays” that “all the qi and chemistry of the twelve channels of the internal organs must be encouraged by the qi of the liver and gallbladder, so that they can be regulated smoothly without disease.”  2.2 The method of draining liver and qi should be used as the basic method for the whole intervention of tumor treatment: We believe that the method of draining liver and qi should be used in the whole intervention of tumor treatment together with other methods of treatment to benefit patients to the greatest extent. A major feature of malignant tumor pathology is blockage and inaccessibility, and local blockage and inaccessibility will inevitably affect the operation of qi in the whole body. Qi is distributed in all parts of the body, and its basic forms of movement are ascending, descending, exiting and entering. When the movement of Qi is not normal, the pathological process of phlegm, dampness, stasis and heat will arise, which will further aggravate the poor operation of Qi, thus forming tumors in a vicious cycle. Clinically, qi disorders are often reflected in the internal organs based on liver qi stagnation, which then affects the movement of qi in spleen qi, lung qi, heart qi, kidney qi and stomach qi, resulting in a series of pathological changes such as stagnation of blood and phlegm, etc. In addition, various clinical symptoms or disease itself cause emotional and mental discomfort or even agitation and depression, which also affect the qi function of the internal organs, and this process often accompanies the disease all the time. This process often accompanies the disease throughout. Qian Bo Wen also believes that the treatment of tumor should first pay attention to regulating qi, and considers it as the fundamental method of treating tumor, while regulating qi should first focus on draining liver and qi. Therefore, it can be considered that draining the liver and regulating qi is one of the basic methods in the treatment of tumors and should be involved in the whole process of tumor treatment.  2.3 The method of draining liver and qi should be coordinated with other treatment rules to treat tumors: According to the etiology and pathogenesis of tumors, Chinese and Western medicine practitioners have, after more than 50 years of clinical practice, proposed such treatment rules and methods as supporting the root, softening and dispersing knots, clearing heat and detoxifying, activating blood circulation and removing blood stasis, attacking toxins with toxins and draining liver and qi. Because of the complex etiology of malignant tumors, which can be roughly summarized into six aspects: qi, blood, phlegm, stagnation, toxicity and deficiency, and not due to a single etiology or pathogenesis, and other pathological factors arising from the imbalance of qi, the rule of draining liver and qi should be appropriately applied clinically in combination with other treatment methods. Under the guidance of the basic principles of “balance yin and yang, holistic treatment”, “identify the symptoms and root cause, weigh the urgency”, “support the righteous and eliminate the evil, stage treatment”, “treat different diseases together, treat the same disease differently”, focus on the combination of disease identification and evidence identification, the combination of the whole and the local, fully flexible and appropriate use of the currently known Under the guidance of the basic principles of “different diseases, same treatment, different treatment”, we focus on the combination of identification and diagnosis, the combination of overall and local, and the full flexible and appropriate use of the known treatment methods and rules to achieve the best individual treatment. For example, in cases of Qi stagnation and blood stasis, Curcuma longa, Sumac, Peach kernel, Safflower, Red peony, etc.; in cases of Qi stagnation and phlegm coagulation, Radix et Rhizoma ginseng, etc.; in cases of Qi stagnation and Qi deficiency, Radix et Rhizoma astragali, Radix prunus ginseng, etc. However, how to make the various treatments justified and appropriate combination is one of the priorities of clinical research today, and clinical research units should be encouraged to actively carry out large sample RCT or partial RCT clinical research in order to provide strong clinical evidence for evidence-based medicine.  3, clinical application of liver regulating method Clinical practice has observed that cancer patients generally have emotional disorders, and those who have suffered major trauma and have been depressed and worried or pessimistic, short-tempered and sentimental for a long time are prone to tumor, and many patients are in a depressed state and have serious depressive symptoms after the occurrence of tumor, which is also closely related to the recurrence and metastasis of tumor. Now we summarize that liver cancer, breast cancer, gastric cancer and lung cancer are clinically applied by the rule of draining liver and Qi.  3.1 Liver cancer Liver cancer belongs to the category of “accumulation”, “jaundice” and “dysmenorrhea” in Chinese medicine, and liver depression and spleen deficiency is a common type of liver cancer. Pan Minqiu [4] analyzed the frequency and ranking of 253 clinical papers on the treatment of mid- to late-stage primary hepatocellular carcinoma and found that patients with liver depression and qi stagnation accounted for 46.7% of the patients, who often presented with clinical symptoms such as dysthymia, abdominal distension, loose stools, emaciation, swelling of the lower extremities or ascites, a pale, fatty tongue with a white, greasy coating, and a slippery pulse. Some scholars believe that spleen deficiency and liver depression is the basic pathogenesis of liver cancer, and the treatment should be based on draining the liver and strengthening the spleen and regulating the qi flow [5], which is in line with the principle of “When we see the disease of the liver, we know that the liver transmits the spleen, so we should first strengthen the spleen” in the Jin Kui Yao. Medicinal uses: Qing Pi, Chai Hu, Yu Jin, Neem, Leech, Yuan Hu in vinegar, Nan Xing and Han Xia. Some scholars have also adopted the method of tonifying the liver and kidneys (Chai Hu, Guang Yu Jin, Chuan Xiong, Xiang Shen, Fructus Fructus, steamed cornelian, Chinese wolfberry, He Shou Wu, Sang Sang Sang, Xian Ling Spleen, Centipede, Licorice) according to the theory of “liver and kidney homologation” to treat 81 patients with significant improvement in clinical symptoms and immune function, thus achieving prolonged survival. Modern clinical research proves that the combination of chemical arterial perfusion and interventional treatment with liver and spleen-strengthening herbs before treatment can improve patients’ immediate and long-term efficacy, quality of life, immune function, blood coagulation status, clinical symptoms, improve immune function, protect liver function and bone marrow function, reduce AFP and improve survival rate, etc.  3.2 Stomach cancer Stomach cancer belongs to the category of “stomach and epigastric pain” and “regurgitation” in Chinese medicine. In clinical practice, the commonly used formulas to regulate liver and qi include Prosperity San, Four Reverse San, Chai Hu Dredging Liver San, Xiang Su San, Yue Ju Wan, Liang Fu Wan, etc. In specific application, attention should be paid to the selection of common drugs introduced into liver meridian or spleen and stomach meridian, such as Citrus Aurantium, Chen Pi, Citron, Buddha’s Hand, Yu Jin, Pregnancy, Hop Huan Pi, Chai Hu, Chuan Hu, Yan Hu, Hou Pu, Citrus Aurantium, Daji Pi, Juxiang, Sha Ren, Xiang Shen, etc. Also, according to the condition and hostages, appropriate combination with strengthening the spleen and regulating qi is required. According to the different conditions of the disease, the prescriptions should be combined with the methods of strengthening the spleen and Qi, clearing heat and detoxification, activating blood circulation and removing blood stasis, softening phlegm and softening hardness, and harmonizing the stomach and eliminating food.  3.3 Breast cancer Breast cancer is mostly described as “breast rock” in ancient literature, such as “Danxi Xinfa”, which says: “A woman’s depression and sorrow are accumulated over time. It is not painful but not itchy, and after ten years, the sore will be trapped, which is called breast rock.” According to the book of “The Authentic Book of Surgery”, “depression hurts the liver, thinking hurts the spleen, and the accumulation of thoughts in the heart, which cannot be achieved, causes the meridians to become gangrenous and nucleated”. Clinical manifestations of breast cancer often include breast lumps, swelling and pain, pain in the two hypochondria, irritability, dizziness, bitterness and dryness of the mouth and throat, thin white or thin yellow coating, and slippery pulse. The clinical use of drugs is often based on the basic pathogenesis of “stagnation of liver qi and deregulation of the flushing and the free flow of Qi”, with the addition and subtraction of Prosperity San, commonly used in the form of Quan Guahu, Chai Hu, Yu Jin, Bai Zhu, Chen Pi, Preparedness Zi and Angelica. In the clinical study, Xu Li et al. used the combination of Chinese herbs with the effect of draining liver and Qi, activating blood circulation and resolving blood stasis, and detoxifying cancer toxins to form a formula based on prolotherapy (Chai Hu, Radix Angelicae Sinensis, Bai Shao, Atractylodes Macrocephala, Poria, Prince’s ginseng, Coix lacrymae, Cyperus rotundus, Psidium guajava, Wang Bu Liu Xing, Curcuma longa, etc.) for the treatment of breast cancer, which was better than that of the chemotherapy alone in terms of improvement of common clinical symptoms and Karlsruhe score.
