What to do with a child’s circumcision as winter vacation approaches?

The foreskin is the skin that covers the outside of the penis and glans. The inner epidermis of the foreskin and the skin of the glans are stuck together, a condition that prevents the foreskin from turning over and fading away, a phenomenon known as physiologic phimosis. Only a few children’s glans can be exposed as freely as adults, so it is normal for children to have phimosis. According to statistics, 90% of newborns have a prepuce, 50% at the age of one and 10% at the age of four. Therefore, before the age of four, about 90% of boys can partially flip the foreskin to fade down, even to the coronal groove below the glans. In other words, physiologic phimosis usually disappears on its own with age, and most boys with phimosis do not experience any uncomfortable symptoms. What is phimosis? Hypercircumcision generally means that the foreskin covers the entire penis and glans, and the glans cannot be exposed naturally, but the foreskin can usually be flipped down to reveal the glans when cleaning. What is phimosis? The phimosis is different from the prepuce. Phimosis means that the opening at the front of the foreskin is too tight and the foreskin cannot be turned down to reveal the glans. When urinating, urine tends to accumulate in the foreskin cavity, making the foreskin expand like a balloon (also known as the “balloon phenomenon”). What are the dangers of circumcision? 1, the accumulation of foreskin scale. 2, infection. When the foreskin glans inflammation occurs, the opening of the foreskin can appear red and swollen, and the penis appears painful and itchy. 3, urination abnormalities. Due to the narrow opening of the foreskin, the urine stream becomes thin and long when urinating, and the foreskin cavity can be enlarged due to the accumulation of urine. Prolonged difficulty in urination and symptoms of lower urinary tract obstruction. 4.Penile cancer may be induced by long-term inflammatory stimulation. 5.Sexual dysfunction. Because the head of penis can not be exposed, the sensitivity is reduced, and it is easy to have delayed ejaculation or non-ejaculation; when the prepuce causes erection pain or intercourse pain, there can be loss of libido, and so on. 6.Circumcision incarceration. After the foreskin is turned up to the coronal groove, the narrowed circumcision is embedded in the coronal groove, so that the venous and lymphatic reflux is blocked, causing edema of the head of the penis and the foreskin, and edema further aggravates the narrowing, forming a vicious circle. If the embedded foreskin is not reset in time, it will cause local infection and ischemic necrosis. What kind of situation must do circumcision? 1.Foreign scholars suggest that if the foreskin is still in place after the age of 5, surgery should be considered. 2.Recurring circumcision, even urinary tract infection, no matter how old you are, you need to operate as soon as possible. 3.When circumcision has occurred. When is it better to do circumcision? Child circumcision is generally not recommended before the age of 8 years for circumcision, this is because the child has not yet normal development, the foreskin is likely to retract on its own, if after puberty, still circumcised or circumcised, before the need for surgical treatment. Circumcision is best performed between the ages of 8 and 16, when a boy’s genitals have grown to a certain degree and he is able to cooperate with the surgery and post-operative care. What type of surgery is best? The most common types of circumcision are circumcision, anastomotic ligation, laser excision of the foreskin, and traditional circumcision. For small children, we usually choose to choose the circumcision ring ligation or circumcision anastomosis ligation; with the traditional circumcision and circumcision laser excision surgery compared to these two surgeries have the following obvious advantages: ① less surgical bleeding (usually less than 5 ml); ② short operating time (usually 5 to 10 minutes); ③ do not need to be stitched up, so that you do not have to bear the pain of removing the stitches after surgery; ④ do not need to lie down after the operation. ④ no need for bed rest after the operation, you can go to school or work after the operation, it will not affect your normal life; ⑤ the possibility of infection is small, after the operation of the routine use of antibiotics for 5 days basically will not appear wound infection; ⑥ the operation is not limited by time, you can do it all year round, and easy to take care of the postoperative period; ⑦ postoperative wound healing neatly and beautifully. How to take care after circumcision? First, circumcision ring 1, from the day of surgery every day with diluted povidone-iodine soak the wound (1 portion of povidone-iodine plus 4-5 portions of cool boiled water diluted), each time for about 3 to 5 minutes, the medicine must cover the entire inner ring (the wound in the back of the inner ring), 2 to 3 times a day, soak with a hairdryer after drying, to keep the wound dry. 2.Post-operative clothing loose pants to reduce wound friction pain; do not sit for a long time with the ring period, long standing, reduce strenuous activities. 3.Take a shower on the 2nd day after the operation, and wash the wound with diluted iodophor and blow dry after the shower. 4, patients with prepuce are mostly accompanied by adhesion, separation of adhesion process will cause partial damage to the foreskin or glans, the healing process may appear more secretion, so the glans and foreskin will be covered with a layer of yellow secretion in a few days after the operation, will slowly become a scab, which is a normal phenomenon. 5.After the operation, you need to wear the ring for 8~10 days, depending on the condition of the wound, you can come to the hospital to remove the ring. On the day of removing the ring, before leaving home or before removing the ring, wash the wound with diluted iodine povidone for 10-15 minutes to eliminate the adhesion between the surgical site and the inner ring, so as to alleviate the pain when removing the ring. After removing the IUD, you still need to pay attention to the wound care, use cotton swabs to wipe with iodized povidone twice a day until the wound is completely healed. 6.Postoperative use of anti-inflammatory drugs can reduce postoperative infection and pain. Second, circumcision anastomosis ligation 1, postoperative recommended oral antibiotics to prevent infection (5 days). 2, the 3rd day after surgery can remove the elastic bandage, use diluted iodophor to soak the wound (disposable paper cups contain one part of iodophor diluted with 4-5 parts of cool water), twice a day, each time for about 3 minutes, after soaking with a hairdryer to keep the wound dry. Note: ① the medicine must cover the entire inner ring (wound behind the inner ring), the day after surgery can be soaked; ② keep dry is very important, usually if the wound is wet, but also with the hairdryer to blow dry. 3.Post-operation must wear loose pants, conducive to ventilation and air permeability, to keep the wound dry and reduce the pain of wound friction; 4.Post-operation day 3 can remove the elastic bandage to take a bath, after the bath must be diluted iodine povidic soak the wound and blow-drying; 5.Children with a small amount of postoperative liquid oozing out of the wound (transparent color, reddish light, yellowish) is a normal phenomenon, such as do not worry about the phone can be consulted by the surgeon, or come to the Department of checkups.
