The process of forming a nursing diagnosis consists of three main stages divided into, analyzing the information, identifying the problem and making a nursing diagnosis. 1. analyzing the information: the person participating in the nursing care has to compare the information and data, categorize the data and test the accuracy and comprehensiveness of the information. 2. Identify the problem: Before making a judgment, the nursing staff makes initial inferences and must understand the patient’s health; determine what problems need to be addressed, whether the application of nursing measures will resolve them, and the range of risk factors and causes of the nursing problem. 3. Make a nursing diagnosis: the final stage is for the nursing staff to make a rigorous comparison of the patient’s expected outcomes and responses to make a diagnosis of the health problem, cause, and signs. During the nursing diagnosis process it is important to provide daily care and autonomy to the patient; the patient should maintain full confidence in the caregiver; and adhere to regular medication as prescribed.
