Nourishing blood and clearing brain granules for the treatment of insufficient blood supply to the vertebrobasilar artery

The concept of vertebrobasilar insufficiency of blood supply (VBI) has been rarely mentioned in foreign medical literature and is more often replaced by the term posterior circulation ischemia (PCI). However, VBI is still widely used as a diagnostic term in China, both clinically and in academic communication. Clinical treatment mostly uses antiplatelet and vasodilator drugs, and our hospital has applied blood-raising and brain-clearing granules to treat 164 patients with vertebrobasilar artery insufficiency from 2005 to 2006 with remarkable efficacy, which is reported as follows: 1. Subjects and methods Subjects: All cases underwent TCD, of which 220 cases were found to have vertebrobasilar artery spasm or ischemia on TCD examination, which is consistent with 88.3% of domestic reports, and 162 cases had positive findings on MRA examination. cases had positive findings on MRA examination, which was in accordance with 53.3% of domestic reports. The diagnostic criteria were based on the following: (1) vertigo; (2) at least one other symptom of vertebral basilar artery ischemia attack, such as visual symptoms, inner ear pain, numbness or weakness of limbs, sudden collapse or fainting; (3) slight signs of brainstem damage; (4) clear etiology: e.g. caused by cervical spondylosis, arteriosclerosis, etc. Methods: Treatment The treatment group was given aspirin, simvastatin, vincristine and antihypertensive treatment. In the control group, the combination therapy was applied alone. Efficacy assessment criteria See Table 1. judged according to the disappearance of various symptoms and TCD results HAMA score results after 72 hours of treatment. Statistical methods The measurement data between the two groups were expressed by mean + standard deviation (x+s) The t-test was used for comparison between groups, and the x2 test was used for effective rate. 2, results There were significant differences between the two groups compared with the treatment results See the following table The disappearance of various symptoms and improvement of blood flow in the two groups (cases, %) p<0.05, adverse reactions There were no significant adverse reactions in the treatment and control groups. The concept of vertebrobasilar artery (VBI) is rarely mentioned in foreign medical literature, and is more often replaced by the term posterior circulation ischemia (PCI). However, VBI is still widely used as a diagnostic term in clinical and academic communication in China. VBI refers to the state of inadequate blood supply to the corresponding perfusion area caused by atherosclerosis and plaque formation, vascular stenosis, hemodynamic changes and congenital anomalies in the vertebrobasilar artery. Due to the long-term chronic inadequate blood supply, the collateral circulation is somewhat open, so the disease is generally slow, but the vertebrobasilar artery, as the upstream vessel of the posterior circulation, is the main channel of blood supply to the inferior and superior curtain brain structures, and once the vertebrobasilar Once vertebrobasilar artery thrombosis occurs, the morbidity and mortality rate is as high as 20%-30% [3]. Therefore, it should be given high clinical attention. Most of the clinical treatment is combined with aspirin, simvastatin, vincristine and antihypertensive. Nourishing blood and clearing brain granules are derived from the classical formula Siwu Tang, which consists of Angelica sinensis, Chuanxiong, Bai Shao, Shu Di Huang, Hooked vine, Chicken blood vine, Xia Ku Cao, cassia seeds, mother of pearl, Yan Hu Suo and Hsio Xin, etc. It can significantly reduce vascular resistance, relieve vascular spasm, increase cerebral blood flow and improve cerebral circulation; reduce platelet surface activity, inhibit platelet aggregation, reduce blood lipids and blood viscosity, prevent thrombosis, and nourish the heart and calm the mind It also has a calming effect on the mind. It can effectively treat the symptoms of dizziness, headache, insomnia and dreaminess, numbness and weakness of limbs caused by VBI, and no toxic side effects were found, with good compliance and clinical effect, which is worthy of clinical promotion.
