Are twice a year infusions really that amazing?

  When spring comes, many middle-aged and elderly patients with cardiovascular and cerebrovascular diseases start to visit various hospitals again, repeating one thing: asking for “infusion” to unblock blood vessels in exchange for six months of physical peace.  I don’t know when this twice-yearly “infusion therapy” became popular nationwide and became the firm belief of many middle-aged and elderly patients; what’s more, it became a winning strategy for many grassroots hospitals to recruit patients. Is it really so amazing that twice a year infusion can prevent cardiovascular and cerebrovascular diseases for half a year? After analysis, we can know.  In terms of efficacy, the common intravenous route to unblock blood vessels are generally drugs with anti-platelet coagulation, expansion of cardiovascular and cerebrovascular arteries, improve microcirculation, such as western drugs such as Chuanxiongzine, Clindamycin, Tianmain, Prostil, Edaravone, butylphthalein, etc., Chinese preparations compounded salvia, bitter dish, Shuxinin, lanugenin, etc. These drugs are not different in nature from other oral drugs except that the route of administration is infusion. Therefore, these preparations are equally effective as oral medications, except that they have a faster onset of action and have an irreplaceable role in rapidly controlling symptoms and preventing disease progression in acute cases. However, for the development of chronic diseases, these infusion preparations do not show their advantages, the use of their treatment is “anti-aircraft gun to hit the mosquitoes” is a big material.  Second, these preparations belong to the drug, it can not be separated from the characteristics of the drug, that is, all have a clearance half-life. The so-called clearance half-life (referred to as half-life) is the time required for the drug to enter the organism through metabolism and reach half of the concentration in the blood, generally after a few half-lives the drug will have no effect, which is why patients need to take the drug several times a day. In the infusion preparations, there is also the problem of half-life, for example, the half-life of western medicine preparation Chuanxiongzin is 1.69 hours, the half-life of asparagine is 4.44 hours, the half-life of clindamycin is 30 minutes, the half-life of edaravone is 2.3 hours, none of them can reach half a year; the half-life of Chinese medicine preparation compound Danshin is 15 hours, the half-life of Shuxin is 2.17 hours, and the half-life of lanosterol injection is 21.6 hours, and none of them None of them can last for half a year. So it seems that the so-called “effective” can only cover the period of infusion. In order to ensure that the disease does not occur for half a year, it is necessary to have a daily uninterrupted infusion to achieve. The claim of twice a year infusion is not only not miraculous, but not reliable at all, and is a “beautiful pie”. It can only be a good wish of the patient, but in practice it does not work.  From the safety point of view, the long-term repeated intravenous infusion of blood vessel stimulation damage is relatively large, because the needle piercing and the stimulation of the drug will cause damage to the wall of the blood vessels, easily cause phlebitis, serious can cause venous thrombosis, secondary to new vascular disease. For patients with heart failure, too much and too fast infusion will increase the blood volume and heart burden, causing limb swelling or pulmonary edema. Moreover, the intravenous drug directly into the blood, there is no gastrointestinal buffer, if once the allergic reaction will be rapid and serious, can be directly life-threatening.  Thus, it seems that intravenous infusion is a double-edged sword, which can save lives if used well, but is harmful to health if used otherwise. Therefore, we advocate the correct preventive treatment method which is to take oral medication on a daily basis. In China, any of the authoritative medical guidelines such as “Coronary heart disease prevention and treatment guidelines”, “Stroke prevention and treatment guidelines”, “Hypertension prevention and treatment guidelines”, “Diabetes prevention and treatment guidelines”, etc., are also used. Diabetes prevention and treatment guidelines” also recommend oral medication, not intravenous infusion listed in them, which should clearly answer this question. In fact, the correct way to prevent and treat cardiovascular disease is to take oral medication every day.
