Chinese medicine calls atopic dermatitis as four bends of wind, and believes that its pathogenesis mainly has 2 factors: internal and external. The internal factor, namely the physical factor, is the root cause of the disease. Patients mostly show endowment intolerance, spleen and stomach disorders, muscle thinning table deficiency; external factors are the triggering factors of the disease, such as feeling wind, damp and heat, eating fish and fishy hair, etc. The underlying pathogenesis of four bends of wind is spleen deficiency and dampness, generating wind, heat and dryness. Treatment should follow the principle of treating the symptoms of the disease urgently and treating the root of the disease slowly, with the acute attack period to clear heat and dampness, dispel wind and stop itching; the chronic period to strengthen the spleen and remove dampness, nourish blood and moisten dryness. 1, Chinese medicine diagnosis and treatment (1) damp-heat skin evidence Most often seen in children and adults in the acute phase, the lesions are mainly erythematous, red papules, blisters or papules, {heat itching, water immersion, or vesicles crust, conscious itching; body heat, disturbed thirst, dry stool, urine yellow red, red tongue, yellow greasy moss, smooth or slippery pulse. After scratching, vesicles appear, a small amount of oozing, crusting, dry and yellow urine, red tongue, thin yellow coating, slippery pulse. The treatment is to clear heat and dampness; children should be treated by eliminating wind and guiding redness, while adults should be treated with addition and subtraction of gentian and liver diarrhea soup. Commonly used drugs in children are 15g of Radix et Rhizoma, 6g of Huang Lian, 6g of Scutellaria, 5g of Mouton, 10g of Poria, 10g of Zelig, 6g of Che Qian Zi, 10g of Jinyin Hua, 10g of White Moss Peel, 6g of Licorice; in adults, 6g of Gentiana, 12g of Scutellaria, 6g of Gardenia, 15g of White Moss Peel, 15g of Atractylodes, 15g of Zelig, 10g of Chai Hu, 10g of Radix et Rhizoma, 6g of Licorice. In case of severe itching, add mother-of-pearl and magnet; in case of poor appetite, add patchouli and pelargonium; in case of dry stool, add daphyllum leaf and hemp; in case of pustules, add honeysuckle and forsythia; in case of dry stool, add rhubarb. (2) Spleen deficiency and dampness are most often seen in infants and young children with a thin constitution, often with recurrent attacks, sometimes light and sometimes heavy, and persistent; the lesions appear as multiple papular rashes, dull in color, with a small amount of ooze and thinness, or with yellowish crusts, or mainly crusted and infiltrated patches, with self-conscious itching; often accompanied by fatigue, fullness of the stomach and abdomen, dullness and loose stools, mucousy mouth, light tongue, white greasy coating, moist and slow or slippery pulse. The treatment is to strengthen the spleen and dampness, moisten dryness and relieve itchiness; the formula uses Jianshu De-Dampness Tang or De-Damp Stomach Ling Tang plus reduction. Commonly used drugs are 10g of Atractylodes macrocephala, 10g of Atractylodes macrocephala, 10g of Magnolia officinalis, 10g of Pericarpium chenopodium; 6g of Chuan Hou Pu and 10g of Pericarpium macrocephala are added for abdominal distension. After the symptoms are basically relieved by the treatment with tonics, Chinese medicines such as Ginseng and Atractylodis Macrocephalae and Ginseng and Spleen Pill can be used for post-treatment. (3) Yin deficiency and blood dryness is seen in patients with long duration of disease in all stages, with mild infiltration of skin lesions; or the affected area is scattered papules, scratch marks, blood scabs, etc.; accompanied by intense itching, with dark red tongue with little fluid, little moss and sunken string veins. Treatment is to nourish Yin and moisten dryness to relieve itching. The formula is based on Nourishing Blood and Moistening Skin Drink and Nourishing Yin and Removing Dampness Soup. Commonly used drugs are Angelica sinensis 10g, Radix Rehmanniae 15g, Radix Rehmanniae 30g, Astragalus membranaceus 10g, Asparagus macrocephala 10g, Macrocephalae 10g, Rhizoma smallpox 10g, Peach kernel 10g, Safflower 10g, Salviae Miltiorrhizae 10g, Radix Bupleurum 15g. If hypertrophic mossiness is obvious, you can add Wang Bu Liuxing, Ze Lan; night restlessness and itching, light use of sour date palm, Hopi skin, nightshade vine, Yuan Zhi, etc. to calm the mind and relieve itching; heavy use of dragon bone, ochre, oyster, magnetic stone to regain calmness and relieve itching. 2, Chinese medicine external treatment hair infant rash vesicles, exudation is obvious to wet compresses, available amaranth, cypress and other water decoction, cold wet compresses; rash exudation is less, can choose three yellow lotion external application, or with Qing Dai San plus licorice oil transfer coated. For children and young adults, the rash is mainly papular, crusty and mossy, so use yellow zinc oxide oil, 3%~5% black bean distillation oil ointment, Qing Dai cream, etc.; for dry skin, use moist muscle cream, Huang Bai cream for external application. The most important thing is to avoid the use of bitter and cold products; if the bitter and cold is too much, it will easily damage the spleen and aggravate the disease. It is advisable to use products that awaken the spleen and eliminate dampness, such as Atractylodes macrocephala, Poria, Yam, Citrus aurantium, Coix seeds, etc. The chronic phase is mostly due to long term disease depletion of yin and blood, or prolonged use of percolation, or excessive use of bitter dryness, resulting in injury to yin and dampness of the evidence, manifested as dry and flaky skin lesions, thick and chapped. The treatment is to nourish yin and remove dampness, commonly used Shengdi, XuanShen, ZhiMu, MaiDong, TianDong, YuZhu, XuanShen, BaiShao, Angelica, BaiMossy, bitter ginseng, etc. In the identification of evidence should pay attention to the relationship between the whole body and the local; in the treatment should be used to take into account both the symptoms and the root cause, to support the righteousness and eliminate the evil, in order to achieve better results.
