What is ovulation The date of ovulation in women is usually about 14 days before the next menstrual period. The egg can survive in the fallopian tube for 1 to 2 days after being discharged from the ovary to wait for fertilization; the man’s sperm can maintain fertilization ability in the woman’s reproductive tract for 2 to 3 days, so it is easy to conceive during intercourse in the days before and after the egg is discharged. To be on the safe side, we refer to the 5 days before and 4 days after the day of ovulation, together with the day of ovulation, as the ovulation period. Ovulation is the period when the follicles are mature and being expelled, when estrogen levels are at their peak and the endometrium is transformed from the proliferative phase to the secretory phase. When the follicle ruptures and is discharged, the estrogen level temporarily decreases and the endometrium loses its hormonal support, resulting in partial shedding and vaginal bleeding. Ovulatory bleeding Ovulatory bleeding refers to a small amount of vaginal bleeding in the middle of two menstrual periods, around the 12th to 14th day of the cycle, accompanied by varying degrees of abdominal distension, lumbar acidity, increased leucorrhea and other symptoms. It is called periovulatory bleeding, also known as intermenstrual bleeding. The amount of bleeding during ovulation is very small, less than normal menstruation, some of them are only coffee-colored discharge, and they occur in periodic episodes, usually stopping on their own in 2 to 3 days, with the longest period not exceeding 7 days. It usually stops on its own in 2 to 3 days and lasts for more than 7 days. It is most common in adolescent and fertile women. Etiology The exact cause of ovulatory bleeding is not well understood so far. 1. After the rupture of the mature follicle, the estrogen level in the body drops sharply and cannot maintain the growth of the endometrium, causing local breakdown and shedding of the superficial layer of the endometrium and bleeding. 2. The mature follicles secrete more estrogen during ovulation, resulting in endometrial congestion and oozing of blood. 3, When the umbilical end of the fallopian tube takes in the egg, the blood-containing follicular fluid is sent retrograde through the fallopian tube to the uterine cavity and then flows out from the vagina. And factors such as excessive exercise, exertion and mental stress can be a major trigger for ovulatory bleeding. Diagnosis 1. Other diseases of the reproductive tract must be excluded, such as menstrual disorders, cervical erosion, cervical polyps, cervical cancer, endometrial polyps, submucosal fibroids, endometrial adenocarcinoma, etc. 2. Its diagnosis needs to be made by examining typical symptoms, basal body temperature biphasic and endometrial biopsy showing endocrine response at least 2d behind. 3. Bleeding occurs around the time of ovulation, with negative cervical screening and no abnormalities found in the uterine adnexa on ultrasound. Treatment 1.If the bleeding is small and short, or if the bleeding is occasional, it usually does not require treatment and can mostly heal on its own. 2.Start with hexestrol 0.5mg, qd, orally 2 to 3 days before ovulation until bleeding stops. To prevent fluctuation of the endometrium, so that it can quickly return to normal, while achieving hemostasis. 3. In case of bleeding, take Enrobam 5mg, tid, vitamin E gel 50mg, qd, orally, and if the bleeding is heavy with the use of haemostatic injection treatment. 4, endometrial exfoliation bleeding, bacteria, viruses, chlamydia and other pathogens are easy to invade and cause infection, so should be combined with antibiotic treatment, bleeding stopped that stop the drug. 5.Keep optimistic emotion, stable heart, adjust mental stress, avoid overwork, etc.. And prohibit the bleeding period of sexual life. It is recommended to consume more beans, milk and other foods to supplement the shortage of estrogen and maintain ovarian function. Pay attention to rest during ovulation, keep local cleanliness and prevent infection; if accompanied by abdominal pain, apply heat to the abdomen. Strengthen physical exercise to enhance physical fitness.
