The sinus tract pus drains and the periapical inflammation does not heal itself, this sinus tract drainage is usually a form of periapical inflammation, where there is a gradual transformation of an abscess at the root tip into a chronic periapical inflammation. Sinus tracts can exist on the surface of the gums, forming localized pus pockets, which is why many patients visit the clinic. If the resistance decreases and the local inflammation increases, pus is discharged. This chronic periapical inflammation cannot be cured without root canal treatment, and may continue to develop into apical granulomas, or apical cysts, or it may flare up acutely to form acute apical abscesses. The only way to heal this condition is to close the pathway for bacteria to enter the root apex through a thorough root canal treatment, along with anti-inflammatory treatments.
