Postoperative patients with esophageal cancer often suffer from intractable diarrhea. Conventional anti-infective, biologic and antidiarrheal treatments often achieve little effect, affecting patients’ postoperative recovery and quality of life, and in severe cases, even leading to severe malnutrition and imbalance of water, electrolytes, acid-base, directly affecting the prognosis of the patients. The use of traditional Chinese medicine internal and external combined treatment method to treat this disease has achieved better results, and one case is introduced as follows: Zhang※※, female, 78 years old, teacher. Zhang※※, female, 78 years old, teacher, came to the clinic in November 2008 because of “diarrhea and watery stools for more than half a year after esophageal cancer surgery”. More than half a year ago (May 2008), the patient was diagnosed with esophageal cancer and underwent resection at a cancer hospital due to progressive dysphagia, with no postoperative radiotherapy. After the surgery, the patient suffered from diarrhea, more than ten times a day, with watery stools. After being treated with anti-infective drugs, biologics and antidiarrheal drugs for more than 40 days without improvement, he was transferred to a tertiary hospital. The patient was given all kinds of western medicines for more than four months, which were ineffective. During the hospitalization period, the patient also sought help from various Chinese medicine practitioners, and the treatment was ineffective in a number of Chinese medicine hospitals. In November 2008, he was referred to a hospital for treatment. At the initial diagnosis, the symptoms were: diarrhea, watery stools, more than ten times a day, stomach and intestines vibrating like thunder, vomiting a lot of watery phlegm and saliva, extreme emaciation, minimal food intake, yellowish color, cold limbs, chest and abdomen cold, tongue with light moss, black and moist, and thin stringy pulse. The diagnosis was weak spleen yang in phlegm-drinking disease. Treatment to warm the spleen to resolve the drink, astringent intestines to stop diarrhea, the formula with gui shui li zhong pills plus flavor: 30g of epiphyllum on the gui 3g of codonopsis 30g of atractylodes 10g of dry ginger 3g of licorice 3g of ginger 3g of ginger 3g of red stone resin 30g, 2 days a dose; burnt hawthorn 30g of cardamom 10g of cardamom 10g of charcoal 10g of bone; moxibustion of water of the heavenly center and guanyuan qihai zhongji alternately once a day. The next day, the patient reported that the stool only once, the abdomen slightly gas over the sound of water, phlegm and saliva in the mouth to reduce; the third day, the patient has a feeling of hunger, can eat a small amount of liquid diet, feces in the stool; the second dose of the family forgot to add ginger, the patient’s bowel movements four times a day, a little recurrent; the second diagnosis of the patient’s spirit turned good, slightly reddish, slightly diarrhea, vomiting a small amount of sparse white foamy phlegm, the tongue is pale moss is slightly yellow and moistened, the pulse string fine. The first dose of the patient’s symptoms is significantly reduced, indicating that the dialectic is accurate, the treatment is effective. Treatment on the basis of the original formula to slightly strengthen the power of warming the spleen. Medicinal: 50g of sliced epiphyllum, 3g of cinnamon, 30g of codonopsis, 9g of ginger, 15g of atractylodes, 6g of licorice, 30g of hematite, 30g of red ginseng, 30g of cardamom, 10g of cardamom, 100g of fennel, 30g of cornelianthus annuus, 30g of clove, 30g of ginger, 30g of ginger, external ironing of the abdomen, and moxibustion, the same as before. From the next day onwards, the patient was able to have one bowel movement per day, which was basically shaped, with increased appetite and reduced cough. At the third consultation, the patient’s spirit improved significantly, the face had a rosy color, stool regulation, abdominal pain, Na can, tongue pale moss white greasy, pulse is stronger than before. This indicates that the deficiency of spleen yang has been reduced, so I will reduce the warm and dry products, and add the medicines for tonifying spleen qi. 30g of Chinese medicinal herbs, 15g of lentils, 30g of epiphyllum, 30g of cinnamon, 3g of Codonopsis pilosulae, 30g of ginger, 3g of atractylodes macrocephala, 10g of licorice, 3g of hematite, 15g of lotus rice, 15g of cardamom, 3g of cardamom, one dose per two days. The patient’s condition continued to improve, and there was a small recurrence due to eating more pork, which was improved after being given lemongrass root and cranesbill soaked in water. Later, the patient’s condition was stabilized, and he was given medicine to return to his hometown in Guangyuan. The patient was an old man, who had stubborn diarrhea after esophageal cancer surgery, and was ineffective after multiple treatments by Western medicine and Chinese medicine, and was treated with Chinese medicine in an integrated way to achieve miraculous results. The reasons are analyzed in the following aspects: 1. Accurate diagnosis and combination of disease evidence: the patient is usually strong, but now his bones are as thin as firewood, and the sound of gastrointestinal water is like thunder. The “Golden Chamber” said: “the person is full of this thin, water away from the intestines, leaching sound, called phlegm drink” patients due to old age, serious illness and surgery and other factors such as damage to the spleen yang, resulting in the spleen’s warmth and transport dereliction of duty, the transfer of the right, the water and grains are not from the positive chemistry, but for phlegm drink, water and drink obstruction, so the water in the intestines, such as thunder; the Qing Yang does not rise, the spleen transport is not healthy, so less food and diarrhea; qi and blood, the water and the blood, the water and the water, so the water is like thunder; Qing Yang does not rise, the spleen transport is not healthy, so less food and diarrhea. When there is no rising of clear yang and unhealthy spleen movement, there is little food and diarrhea; when there is no source of qi and blood biochemistry, there is emaciation. Tongue pale moss black moist, pulse string thin also for Yang Qi deficiency, water-dampness phlegm-drink internal phenomenon. Taken together, the patient’s diagnosis should be weak spleen yang in phlegm-drinking disease. 2. Prescription and medication should follow the evidence: therapeutically, “for phlegm-drinking disease, warm medicines should be used to harmonize the disease.” Therefore, the patient’s treatment is based on the principle of warming. This case is a symptomatic case of deficiency and the deficiency is the main cause, therefore, warm yang should be used to stabilize the root, warm the spleen and resolve the phlegm and drink. At the early stage, Ling Gui Zhu Gan Tang and Lizhong Pill were slightly insufficient in strength, so the formula was Gui Hui Lizhong Pill plus Chi Shi Li Wan to warm the middle Jiao and disperse the cold, benefit the qi and strengthen the spleen, and astringe the intestines to stop the diarrhea. After the typhoid fever, it is said, “If the water and cold win over each other, it is when the spleen and kidney double tonic, and add appendages, then the life gate benefit, the mother of the earth is warmed up.” After the second diagnosis, the patient’s tongue from the tongue light moss black moist change to the tongue light moss yellow moist and then change to the tongue light moss white greasy, the pulse is gradually strong, indicating that the gradual reduction of yang deficiency, phlegm and drink gradually warming, treatment that is from warming the spleen and kidney to warming the spleen and yang to tonify the spleen and qi and then to strengthen the spleen and stomach, gradual and orderly transition, the patient’s condition that presents a continuous and steady trend of improvement. 3, cinnamon and red stone resin must be used, the opposite of each other: cinnamon is hot and pungent, to assist the epiphyllum, tonifying fire to help Yang. Traditionally, cinnamon and chrysoprase is one of the “nineteen fears” and should not be paired. Cinnamon and chrysoprase are both sweet and hot products, and cinnamon flavor, chrysoprase taste astringent, the two drugs match, cinnamon when the pass can not pass, chrysoprase when the astringent can not be astringent, each other to reduce the efficacy of the drug. But modern pharmacological research shows that the red stone fat mainly contains aluminum hydrosilicate, through the adsorption of toxic and harmful substances in the gastrointestinal tract, to protect the mucosa of the digestive tract and play the role of astringent antidiarrheal effect; cinnamon mainly contains volatile oils, through the improvement of intestinal microcirculation, promote intestinal motility, increase secretion of the digestive tract, enhance the digestive function, remove the digestive tract, and the inhibition of intestinal pathogenic bacteria pathway antidiarrheal. Both from a different perspective, different ways to play a role, can be said to be the opposite of each other, the same way. To a certain extent, this suggests that the use of cinnamon and red stone fat is not an absolute contraindication; on the contrary, if used properly, can cure the disease. The successful pairing of cinnamon and red stone resin in this case also suggests that we need to carry out more experimental research in this area, to re-verify and organize the traditional contraindications in the motherland medicine, in order to guide the clinical use of drugs.
