The physiological changes of human body have biological cycle, under the control and regulation of biological clock, many biological indicators of human body such as body temperature, blood sugar and hormone level have rhythm and peak and valley value within 24 hours, human blood pressure also has this biological rhythm, according to the different biological rhythm, blood pressure can be divided into spoon type hypertension and non-spoon type hypertension. Most people’s blood pressure is spoon type hypertension, that is, the blood pressure fluctuates in a state of “two peaks and a valley” within 24 hours, from early in the morning, the systolic pressure rises rapidly by 20-50 mmHg and the diastolic pressure rises by 10-15 mmHg, with two peaks occurring at 9-10 and 16-18 hours respectively, while from 18 hours onwards it starts to slowly The blood pressure decreases slowly from 18:00 to 2:00 a.m. and reaches its lowest level at 2:00 to 3:00 a.m. The blood pressure decreases by 10% to 20% from the daytime peak. When blood pressure reaches its peak, patients often experience dizziness and headache, and are prone to cerebral hemorrhage, so hemorrhagic strokes tend to occur during the day; while when blood pressure is at its lowest, cerebral artery blood supply is easily inadequate, and blood flow is slow to form cerebral thrombosis or coronary thrombosis, so ischemic strokes tend to occur at night. Therefore, the “two peaks and one valley” period is a potential risk period for hypertensive patients. To effectively and smoothly control the blood pressure of patients with spoon type hypertension, long-acting antihypertensive drugs such as amlodipine, valsartan, carvedilol, ramipril, lacidipine, and compound antihypertensive tablets should be taken only once a day at around 7:00 a.m., or twice a day at 7:00 a.m. and 3-6:00 p.m. Patients with spoon type hypertension should not take antihypertensive drugs at bedtime or at night, as they are prone to hypotension. Blood pressure is too low, resulting in insufficient blood supply to the cerebral arteries and inducing ischemic stroke. However, there are a few people whose blood pressure decreases by less than 10% at night or by more than 20% during the day, and whose blood pressure curve is non-spoon shaped, called non-spoon hypertension. For people with non-spoon type blood pressure, however, they should take their medication at night before bedtime. Research results show that most of the available drugs are anti-hypertensive drugs with long-lasting effects, such as perindopril, metoprolol extended-release, carvedilol, amlodipine, diltiazem extended-release, valsartan, etc. This can make the peak plasma concentration of the drug and the peak of blood pressure basically synchronize or meet, so as to achieve the ideal effect of lowering blood pressure. Therefore, it is very meaningful for hypertensive patients to monitor their blood pressure, understand their type of hypertension and consult their physicians or pharmacists to determine the timing of medication according to the biorhythmicity of their blood pressure for effective and smooth blood pressure control.
