What to do if a hemiplegic patient can’t bend the hip or knee?

Patients with hemiplegia are unable to flex the hip and knee mainly because of the insufficient separation movement of the knee joint and the relative incoordination of the muscle groups around the knee joint, which leads to the decrease of the stability of the knee joint, and the treatments mainly include the weight-bearing training of the affected limb, proprioceptive training, knee joint control training, and other treatments. 1. Weight-bearing training of the affected limb: instruct the patient to take the healthy limb off the ground, and carry out small-scale alternating flexion and extension training of the knee joint of the affected limb, during which the affected limb should be kept in 15°flexion for 3-5s, and then it should be straightened and maintained in left and right flexion for 5°, and at the same time the center of gravity should be placed in the lower limb of the affected side, so as to prevent the knee from over-extension caused by the forward leaning of the torso. 2. Proprioceptive training: take the supine position, the rehabilitation instructor will stimulate the patient’s knee joint by pulling and squeezing, and gradually increase the strength to promote the recovery of knee joint proprioception. 3. Knee control training: the patient lies prone on the treatment bed, the paraplegic knee does flexion and extension movements, the requirement is not to flex the hip joint when bending the knee, the movement should be slow in the process of flexion, and listen to the therapist’s instructions to achieve control in all angles at any time. 4. Other treatments: mainly include stretching, acupuncture, physical factor therapy, such as wax therapy, etc., which can reduce the muscle tension of the lower limb extensor muscles and promote the emergence of detachment movement. Patients with hemiplegia should actively cooperate with the doctor’s treatment after their condition is stabilized, train under the guidance of the rehabilitation therapist, and refrain from walking without authorization, so as not to aggravate the abnormal movement pattern.
