Abstract: Hepatic hemangioma is a common benign tumor of the liver. In this case, the patient, a 57-year-old female, had suffered from hepatic hemangioma for many years, and the tumor began to grow faster last year, and then came to our hospital for consultation. After discussing with the patient and her family, we decided to give surgical treatment to solve the current problem and prevent the tumor from continuing to grow. The surgery went smoothly and the tumor was completely removed with less trauma and less impact on the body, and the patient recovered very quickly after the surgery.

I. Initial consultation
Hepatic hemangioma is a disease frequently encountered in outpatient clinics. In this case, the patient found hepatic hemangioma for 1 year, and the size of hepatic hemangioma was only 5cm when it was first found, but it has grown to 8cm on review this year. Therefore, I explained to the patient that the indications for surgery are relatively lenient, and if the hepatic hemangioma is growing fast, early surgery is recommended. The smaller the size of the tumor, the less difficult it is to operate, while the larger the size of the tumor, the more difficult and traumatic it may be to operate. After explaining some relevant information to the patient, the patient decided to be hospitalized for surgery.
II. Treatment process
After admission, the patient underwent an intensive CT examination, which revealed that the hepatic hemangioma was located in the right liver and was compressing important blood vessels, so the patient was given a three-dimensional reconstruction for more accurate surgery. After communicating with the patient’s family about the risks of surgery, the patient’s family agreed to the surgical treatment. After active preoperative preparation, the patient underwent laparoscopic hepatic hemangioma resection under general anesthesia, and the intraoperative bleeding was about 200mL. After the operation, routine anti-infection with sodium penicillin for injection; acid suppression with sodium omeprazole for injection; and anti-inflammation with sodium glucose chloride injection were given, and the patient recovered smoothly after the operation.
III. Treatment effect
Intensive CT examination was done before the operation, and 3D reconstruction was generated according to CT to guide the operation, so the operation was very precise, the bleeding was relatively small, the operation time was relatively short, and the operation was done laparoscopically, so the operation was very traumatic, so the patient recovered especially fast after the operation, and there were no obvious postoperative complications such as abdominal infection, bleeding, biliary fistula, etc. The patient had the abdominal drain removed on the 5th day after the operation, and was cured 7 days after the operation. The patient’s abdominal drainage tube was removed on the 5th postoperative day and he was discharged from the hospital 7 days after surgery. The patient and his family were very satisfied with the treatment, and when he was discharged, he said, “Thank you so much for solving my problem.
IV. Notes
We are glad that the patient’s condition is cured after the treatment. After the patient is discharged from the hospital, he should mainly rest, gradually increase the amount of exercise, gradually increase nutrition, and slowly regain vitality. Generally in about 1 month after the operation, you can return to normal work and life. And patients are advised to come to the hospital for a review once a month after surgery, because sometimes there may be residual infection in the abdominal cavity and other conditions after surgery. Many small hemangiomas are not detected or may continue to grow, so regular checkups can help to eliminate some hidden dangers.
V. Personal insight
Hepatic hemangioma is a common benign tumor of the liver, and in many cases it is found during physical examination. Most patients with hepatic hemangioma are relatively small in size, so they do not need to be particularly nervous at this time, and only need regular review. If the size of hepatic hemangioma does not change much, we can continue to observe and do ultrasound examination once a year. If the hemangioma grows faster or gradually exceeds 10cm, or if the location of the hemangioma is particularly bad, such as near important blood vessels or important pipelines, these cases need to be treated with surgery as soon as possible.
