What is the cause of arm swelling after breast cancer surgery?

  Surgery can lead to local soft tissue injury around the wound, local tissue edema and exudation, and aseptic inflammation, which can compress local blood vessels and lymphatic vessels, thus leading to blood and lymphatic return obstruction (lymphatic return obstruction is more serious in patients with axillary lymph node dissection), so swelling of the affected upper limb may occur after surgery. This is a condition that occurs in every patient after surgery, but the degree of edema manifests itself differently in different patients with different degrees of surgical injury and their own reflux compensation. With the extension of time, the inflammation is gradually absorbed, blood and lymphatic reflux obstruction is gradually reduced, and edema will gradually improve. It has been observed that most patients with edema immediately after surgery can gradually improve on their own within 2 months even without treatment.  If the wound does not heal for a long time after surgery, it means that the local circulation is not good, the inflammation is not easily absorbed, the blood vessels and lymphatic vessels are under pressure for a long time, and the obstruction of blood and lymphatic flow is gradually heavier, resulting in the upper limb on the affected side not swelling just after surgery, but swelling after discharge. After timely and proper treatment, edema at this time is also recoverable. If the treatment of edema is delayed due to the influence of other treatments and the degree of concern, it may develop into chronic, “irreversible” edema, which is very difficult to treat and may further develop into “elephantiasis” and even amputation.  As for the re-occurrence of edema one year after surgery, there are many reasons for it, and as far as treatment is concerned, there is no special method at home and abroad. The most widely used is the pneumatic circulation drive pump (commonly known as the big cuff), which is available in the breast specialist or rehabilitation department of many hospitals; secondly, there are drugs, both Chinese and Western medicines, but there are not many varieties and not all hospitals have them; finally, there is surgery, that is, the broken lymphatic vessels are connected to the veins to promote the return of lymphatic fluid. Of course, there are certain conditions for this kind of surgery.  At present, mainly according to the theory of Chinese medicine meridians, using the method of massage, combined with pneumatic circulation drive pump treatment, patients reported good results, the subjective feeling of some symptoms, such as swelling, pain relief will generally be effective after 3-5 times of treatment, but the reduction of limb edema treatment time is longer, may take several months.  Another point to note is that even if the swelling is reduced through treatment, we should not be careless and should pay attention to the influence of all factors in daily life to avoid recurrence.
