What to do about abdominal pain in mesenteric lymphadenitis

  Mesenteric lymphadenitis is a non-specific inflammatory disease, which is one of the causes of abdominal pain in children, often prone to recurrent attacks, and in recent years there is an increasing trend in the incidence. The diagnosis is mainly based on: 1. history of intermittent non-localized abdominal pain, with pressure pain but no rebound pain, mostly located in the right lower abdomen and umbilicus, which cannot be relieved by antispasmodic drugs.  2.Multiple enlarged lymph nodes were seen on ultrasonography.  3, Color Doppler flow imaging (CDFI) shows significantly increased blood flow in the lymph nodes.  It is often due to viral, mycoplasma or bacterial infection, which enters the local lymph nodes with the lymphatic vessels. Since the mesenteric lymph nodes are mainly distributed around the mesenteric vessels, lymphatic fluid is collected from the lymph nodes distributed along the mesenteric vessels and their branches. The mesenteric lymph nodes in the distal ileum are very rich, and the contents stay longer in the distal ileum due to the action of the ileocecal valve, and toxins and bacterial metabolites are easily absorbed by the mesenteric lymph nodes in the ileum, so mesenteric lymphadenitis mostly occurs in the mesentery of the distal ileum.  Acute abdominal pain accompanied by fever and vomiting requires immediate medical attention, and pain medication should not be taken until a clear diagnosis is made.  Mesenteric lymphadenitis should be differentiated from intestinal spasm, intestinal ascariasis, and appendicitis: 1. Intestinal spasm. Most often seen in infants, recurrent bouts of abdominal pain may occur, with no abnormal abdominal signs, and may be relieved by exhaustion and defecation.  2, intestinal ascariasis. In contrast, there are often symptoms of irregular abdominal pain, paranoia, heterophagia, nausea, vomiting and other digestive disorders, and sometimes systemic allergic symptoms; there is often a history of vomiting and excretion of worms; fecal search for eggs, and effective deworming treatment can assist in the diagnosis.  3, acute appendicitis. Mostly abdominal pain first and then fever: first abdominal pain and gradually increasing and right lower abdominal pressure pain is obvious, pressure point fixed, there is obvious rebound pain and muscle tension, and then there is fever, and the white blood cell count is significantly higher, neutrophils are relatively high, ultrasound can be seen swollen appendix, its surrounding may be accompanied by enlarged lymph nodes.  4. Mesenteric lymphadenitis. Mostly fever followed by abdominal pain: there may be fever and other manifestations of infection followed by abdominal pain, which may be vague or spasmodic, with no discomfort in the child between the two pains.  Intestinal lymphadenitis belongs to the category of abdominal pain in Chinese medicine, the location of the disease is in the spleen and gastrointestinal, the internal cause is blamed on children who are not full of formal qi, lung, spleen and kidney deficiency, the external guard is not solid, easy to touch the poisonous evil, the causative factor is blamed on the wanton consumption of cold, fatty, sweet, greasy products and little vegetables, easy to gather dampness and produce phlegm, the external cause is blamed on the invasion of toxic evil, the pathogenesis is the result of the combined effect of many factors, the pathogenesis is toxic congestion and stagnation of qi, phlegm and dampness, knotted in the abdomen, not pass is pain, not glory If it does not pass, it hurts, and if it is not honored, it hurts. The treatment is mainly based on detoxification and dispersion of toxins, warming and dispersing cold evil, eliminating food and stagnation, warming the middle and nourishing deficiency, activating blood circulation and removing blood stasis.  In the acute stage of mesenteric lymphadenitis, abdominal pain can be significantly improved after symptomatic, anti-infective, traditional Chinese medicine and physical therapy treatment.
