In the past, the only way to treat a child with a congenital right thumb deformity is to remove the thumb and then move the index finger over to be used as the thumb. This option also allows the child to have a functioning thumb, but the appearance of the thumb is not as good, as there are only four fingers, which is unacceptable to many parents. Therefore, we now treat floating bunions mainly from the protection of the five fingers, there are two options, one is metatarsal bone reconstruction, the first metacarpal bone is taken from the foot to reconstruct the first metacarpal bone, more or less on the foot is a little bit of an impact. Therefore, I advocate the use of the second method – semi-metacarpal bone grafting reconstruction technique. We are now basically using this method in the treatment of bunions, taking part of the bone from the second metacarpal bone to rebuild the first metacarpal bone, which has the following three advantages: 1. The whole operation is only done on the hand, and it no longer injures other parts of the body, so that the trauma of the operation is reduced to a minimum. 2. Because the metacarpal bone is taken from the hand, the chance of bone resorption and necrosis is greatly reduced. 3. The age of surgery is advanced to about 6 months to 1 year, which is the stage of thumb function establishment, so the child’s thumb function can be established more smoothly.
